陈无咎衷中参西诊治脾胃病特色探析

Study on the Diagnosis and Treatment Characteristics of CHEN Wujiu with Integrated Traditional Chinese and Western Medicine in Spleen and Stomach Disease

[目的]探析近代浙江义乌名医陈无咎以衷中参西为主要观点的脾胃病诊治特色。[方法]通过研读陈无咎相关著作和他人相关学术研究论文,整理其诊治脾胃病方面的有关内容。从陈氏论述较为详尽的膈证、胃痛、反胃、胃胀、胆枯病证着手,挖掘探讨陈氏对5种脾胃病的病机分析和辨证论治特点,并结合陈氏临证验案分析总结其遣方用药特色。[结果]陈氏论治脾胃病融合中西医理念,观点独到,如以消食祛风法治膈证,消磨食积法治胃痛,实肠虚胃法治反胃,清补并用法治胃胀,清胆和胃治胆枯,并根据不同疾病的病机治法制订痛鬲汤、驱寇方、通胃汤、缩胃饮、清胃饮以及还胆汤验方。根据陈氏医案记载,临床疗效显著。[结论]陈氏在脾胃病诊治方面,秉持衷中参西观点,治法理念独树一帜,遣方用药别具一格,对当今治疗临床治疗类似脾胃病极具借鉴价值。

[Objective]The paper discussed and analyzed the diagnosis and treatment characteristics of modern famous doctor CHEN Wujiu in spleen and stomach disease. [Method]The authors studied CHEN＇s related writings, selected the diseases of diaphragm obstruction, stomachache, regurgitation,gastrectasia and dry gallbladder,then discussed CHEN＇s personal opinions on pathogenesis analysis and syndrome differentiation, concentrated his prescription and drug characteristics in spleen and stomach disease. [Result]CHEN integrated the concepts of traditional Chinese medicine and western medicine,and had some unique ideas in treating spleen and stomach disease,such as aiding digestion and dispelling wind to treat diaphragm obstruction,helping indigestion to treat stomachache,descending stomach and dredging intestine to treat regurgitation,simultaneous application of cooling and nourishing therapy to treat gastrectasia,cleaning gallbladder and harmonizing stomach to treat dry gallbladder.CHEN formulated prescription according to his therapy like Tongge decoction,Qukou decoction,Tongwei decoction,Suowei decoction,Qingwei decoction and Huandan decoction,all of them had good clinical effects.[Conclusion]CHEN upheld ideas of integrating traditional Chinese medicine and western medicine in treating spleen and stomach disease.His therapy was unique,the prescriptions were distinguished,which had reference value to today＇s clinical treatment of TCM.
