子宫动脉下行支结扎对宫缩乏力性产后出血的治疗效果

Effect of descending branch of uterine artery ligation on treatment of postpartum hemorrhage due to uterine inertia

目的探讨分析子宫动脉下行支结扎对宫缩乏力性产后出血的治疗效果。方法选取于2014年1月至2016年12月来浙江省永康市第一人民医院分娩的160例宫缩乏力产后出血的孕妇,将所有患者随机分为观察组和对照组,每组各80例。观察组患者行子宫动脉下行支结扎进行治疗,对照组患者单纯接受宫缩剂进行治疗。对比分析两组患者的不良反应和出血情况。结果观察组患者术后2h出血量和24h出血量显著少于对照组患者(t值分别为-32.817、-47.221,均P<0.001)。观察组中患者总出血量>1L发生率显著少于对照组(χ~2=8.901,P<0.05)。观察组患者的止血时间明显短于对照组患者(χ~2=45.972,P<0.001)。对照组患者术后不良反应发生率为13.75%,显著高于观察组患者(P<0.05)。结论子宫动脉下行支结扎术应用于宫缩乏力性产后出血的治疗中,临床效果明显,止血迅速,无明显不良反应。

Objective To investigate the effect of descending branch of uterine artery ligation on treatment of postpartum hemorrhage due to uterine inertia. Methods Totally 160 pregnant women with postpartum hemorrhage caused by uterine inertia treated in the First People＇ s Hospital of Yongkang from January 2014 to December 2016 were selected and were randomly divided into observation group and control group with 80 cases in each group. The patients in the observation group were treated by descending branch of uterine artery ligation, while those in the control group only received contractions. The adverse reactions and bleeding of two groups were compared and analyzed. Results The bleeding volume at 2h and 24h after surgery in the observation group was significantly less than that in the control group （t value was -32.817 and -47.221, respectively, both P〈0.001）. The incidence of total bleeding volume over 1L in the observation group was significantly lower than that in the control group （χ^2=8.901, P〈 0.05）. The hemostasis time of the observation group was significantly shorter than that of the control group （χ^2 = 45.972, P〈 0.001）. The incidence of adverse reactions in the control group was 13.75%, which was significantly higher than that in the observation group （0.00%） （P 〈0.05）. Conclusion Descending branch of uterine artery ligation in the treatment of postpartum hemorrhage caused by uterine inertia obtains remarkable clinical effect and rapid hemostasis. No obvious adverse reactions are found.
