非手术脊柱减压治疗腰椎间盘突出症的疗效观察及机制探讨 被引量:3

Non-Surgical Spinal Decompression in Treatment of Lumbar Disc Herniation and Its Mechanism Investigation

摘要 目的:观察非手术脊柱减压系统(SDS9800)治疗腰椎间盘突出症的疗效,探讨其疗效与影像学检查结果的相关性及其作用机制。方法:将40例腰椎间盘突出症患者随机分为治疗组和对照组各20例,在常规治疗基础上,治疗组给予非手术脊柱减压治疗,对照组给予普通腰椎牵引治疗,观察两组患者疼痛视觉模拟评分(VAS)和直腿抬高角度(SLR)的改善情况,采用医学图片浏览软件(Radiant DICOM Viewer 4.0.3)测量腰椎MRI突出物面积及突出物与神经根管间的夹角(AN)。结果:治疗组VAS评分、SLR改善程度均显著优于对照组(P<0.05);治疗组AN变化程度显著大于对照组(P<0.05),两组突出物面积均未发生显著改变(P>0.05)。结论:非手术脊柱减压治疗腰椎间盘突出症疗效显著,其疗效可能与神经根和突出物的位置变化有关,与突出物面积变化无相关性。 Objective: To observe the efficacy of non-surgical spinal decompression system(SDS9800) for lumbar disc herniation, investigate the correlation and mechanism between the efficacy and imaging findings. Methods: 40 cases of patients with lumbar disc herniation were randomly divided into treatment group(20 cases) and control group(20 cases), on the basis of conventional treatment, the treatment group treated with non-surgical spinal decompression while control group with conventional lumbar traction, observed the improvement of VAS score and SLR, and measured the protrusion area and angel between protrusion and nerve root canal(AN) under MRI with Radiant DICOM Viewer 4.0.3. Results: The improvement of VAS score and SLR of treatment group were significantly better than those of control group(P〈0.05); the degree of AN change of treatment group was significantly greater than that of control group(P〈0.05), while the protrusion area of both groups had no significantly changed(P〈0.05). Conclu Conclusion: Non-surgical spinal decompression has a significant curative effect on lumbar disc herniation, which therapeutic effect may be associated with the position changes between nerve root and protrusion, and is not related to the change of protrusion area.
