改良牙冠延长术和正畸牵引加牙冠延长术治疗上前龈下牙体缺损的效果观察

Effects of modifed crown lengthening and orthodontic traction combined with crown lengthening in thetreatment of subgingival defect of upper anterior teeth

目的探究改良牙冠延长术和正畸牵引加牙冠延长术治疗上前龈下牙体缺损的效果.方法应用前瞻性研究方法,采用随机数字表法将72例上前龈下牙体缺损患者分为改良牙冠延长术组（A组,n=36）和正畸牵引加牙冠延长术组（B组,n=36）.A组患者予以改良牙冠延长术治疗,B组患者予以正畸牵引＋牙冠延长术治疗方案.观察两组患者治疗前后牙齿松动度（toothmobility,TM）、牙周探诊深度（probingdepth,PPD）、龈沟出血指数（sulcusbleedingindex,SBI）、菌斑指PLI）等指标变化情况,比较两组患者依从性和治疗时间.结果随访6个月后,两组患者TM、PPD、SBI、PLI等指标均较术前降低（P〈0.05）,组间比较差异无统计学意义（P〉0.05）.A组患者治疗依从性高于B组,总治疗时间短于B组（P〈0.05）.结论改良牙冠延长术和正畸牵引＋牙冠延长术均为治疗上前龈下牙体缺损的常见术式,两者疗效相当,但改良牙冠延长术治疗总时间更短,患者依从性更高,应根据实际情况选择合适的手术方式.

Objective To explore the effects of modifed crown lengthening and orthodontic traction combined with crown lengthening in the treatment of subgingival defect of upper anterior teeth. Methods 72 cases of patients with subgingival defect of upper anterior teeth were divided into the modifed crown lengthening group （the group A, n=36） and the orthodontic traction combined with crown lengthening group （the group B, n=36） according to the random number table method. The group A was treated with modifed crown lengthening, and the group B was treated with orthodontic traction combined with crown lengthening. The changes of tooth mobility （TM）, periodontal probing depth （PPD）, sulcus bleeding index （SBI）, and plaque index （PLI） were observed in the two groups before and after treatment, and the compliance and treatment time of the two groups were compared. Results After 6 months of follow-up, the levels of TM, PPD, SBI, and PLI in the two groups were lower than those before operation （P〈0.05）, and there were no statistically signifcant differences between the two groups （P〉0.05）. The compliance in the group A was higher than that in the group B, and the total treatment time was shorter than that in the group B （P〈0.05）. Conclusions Both modifed crown lengthening and orthodontic traction combined with crown lengthening are common procedures for the treatment of subgingival defect of upper anterior teeth, and the two have similar effcacy, but the total treatment time of the former one is shorter and the patients＇ compliance is higher, therefore the appropriate surgical approach should be chosen based on the actual situation.
