鼻内镜下鼻腔泪囊造孔术治疗慢性泪囊炎的疗效观察

Clinical observation of endoscope dacryocystorhinostomy for the treatment of chronic dacryocystitis

目的观察鼻内镜下鼻腔泪囊造孔术治疗慢性泪囊炎的疗效。方法对21例21眼慢性泪囊炎患者,采用鼻内镜下鼻腔泪囊造孔术,术中如发现泪囊条件差,则联合泪道置管术;术后定期冲洗泪道,鼻内镜随访1-3月,并分析其临床疗效。结果治愈18眼(85.7%),好转2眼(9.5%),无效1眼(4.8%),总有效率95.2%。结论鼻内镜下鼻腔泪囊造孔术治疗慢性泪囊炎,手术操作简便,术野清晰,术中可清楚辨识钩突肥大、小泪囊、泪囊结石、泪囊囊肿、中鼻道狭窄等体征并针对性处理,且不损伤皮肤及轮匝肌组织,术后面部无疤痕,保留了泪液泵功能,具有较好的治疗效果。

Objetive To observe the effect of endoscope dacryocystorhinostomy for the treatment of chronic dacryocystitis. Methods 21 cases （21 eyes） with chronic daeryocystitis were treated by endoscopic dacryocystorhinostomy, and combined with lacrimal duct tube insertion if intraoperative lacrimal sac condition is poor. The clinical efficacy was observed by regular irrigation test of the lacrimal duct and endoscopic follow-up of 3 months. Results Eighteen cases （85.7%） are cure, two cases （9.5%） are mend, and one cases （4.8%） is invalidation. The general efficiency is 95.2%. Conclusion Endoscopic dacryocystorhinostomy has good effect in treatment of chronic dacryocystitis, which can clearly identify and targeted process the hypertrophic uncinate process, small lacrimal sac, stone and cyst in lacrimal sac , stenosis of the middle nasal neatus. It possesses several advantages such as simple maniqulation, clear operation field, small tissue injuries, no scar of face and preserve most tear pumping function after operation.
