无创呼吸机应用于COPD呼吸衰竭合并慢性心功能不全的临床观察 被引量:2

摘要 目的:探讨无创呼吸机治疗COPD呼吸衰竭合并慢性心功能不全的价值。方法:收治COPD、呼吸衰竭合并慢性心功能不全患者46例,随机分成治疗组和对照组,各23例。对照组给予常规治疗,治疗组在对照组基础上配合无创呼吸机治疗。结果:治疗后,治疗组PaO_2、SpO_2、pH值、LVEF明显升高,PaCO_2、BNP、HR、RR明显下降,与对照组相比,差异有统计学意义(P<0.05)。结论:BiPAP呼吸机治疗COPD呼吸衰竭合并慢性心力衰竭患者,可纠正呼吸衰竭,改善患者心功能及临床症状。 Objective:To investigate the effect of noninvasive ventilation in the treatment of COPD respiratory failure with chronic heart failure.Methods:46 eases with COPD, respiratory failure combined with chronic heart failure were selected,they were randomly divided into the treatment group and the control group,with 23 eases in each group.The control group were given routine general treatment,while in the treatment group were given non-invasive ventilator therapy based on routine general treatment.The treatment rate of two groups were compared after 1 weeks,blood gas analysis,cardiac function,left ventrieular ejection fraction. Results:After the treatment,the PaO2,SpO2,pH and LVEF increased in the treatment group,while the PaCO2,BNP,HR and RR decreased significantly,and the differences were statistically significant compared with the control group(P 〈 0.05).Conclusion: BiPAP ventilator in the treatment of COPD with respiratory failure in patients with chronic heart failure,not only can correct respiratory failure of patients,but also can improve the heart function and clinical symptoms of patients.
