非小细胞肺癌患者电视辅助胸腔镜肺叶切除术中转开胸的影响因素分析 被引量:19

Risk factor analysis of video-assisted thoracoscopic lobectomy for conversion to thoracotomy surgery in patients with non-small-cell lung cancer

摘要 回顾性分析65例行电视辅助胸腔镜肺叶切除术(VATS)中转开胸的非小细胞肺癌(NSCLC)患者病例资料,选取同期未中转开胸的117例NSCLC患者作为对照。探讨影响NSCLC患者VATS中转开胸的影响因素。单因素分析结果显示:男性患者、年龄≥65岁、肺结核病史、肿瘤位于上肺、肿瘤直径≥35.00 mm、胸膜粘连程度≥4级为中转开胸的影响因素。Logistic回归分析显示:肺结核病史、肿瘤位于上肺、胸膜粘连程度≥4级为中转开胸的独立影响因素。 A retrospective analysis of 65 cases of video-assisted thoracoscopic lobectomy( VATS) transferred thoracotomy in patients with non-small cell lung cancer( NSCLC) data,selected 117 cases of NSCLC patients which the same period did not transfer thoracotomy as a control to explore the impact factor of NSCLC patients with VATS. Univariate analysis showed that male,age≥65 years,tuberculosis history,upper lung tumor,the maximum tumor diameter≥35. 00 mm,pleural adhesion level≥4 were risk factors for conversion to thoracotomy surgery. The results of mltivariate logistic regression analysis showed that tuberculosis history,upper lung tumor and pleural adhesion level≥4 were independent risk factors for conversion to thoracotomy surgery.
