小切口趾长伸肌腱延长术结合趾间关节成形术治疗爪形趾锤状趾96例分析

The analysis of 96 cases of claw-toe mallet toe with long extensor tendon lengthening and interphalangeal joint arthroplasty

目的观察小切口趾长伸肌腱延长术结合趾间关节成形术治疗爪形趾锤状趾的疗效。方法对96例177趾中重度爪形趾锤状趾患者采用小切口延长趾长伸肌腱,松解跖趾关节挛缩关节囊,磨钻磨除近节趾间关节面近远端,磨除适量骨质使其关节成型,手法矫正,纱布绷带包扎,分趾器外固定。患者全部得到随访,随访时间1～5年,手术前后采用桑志成小切口治疗锤状趾评价标准。结果治疗96例177趾,爪形趾锤状趾矫正满意,优良率达94.35%。结论小切口趾长伸肌腱延长术结合趾间关节成形术对中重度爪形趾锤状趾矫正畸形疗效可靠。

Objective To observe the curative effect of small incision toe extensor tendon lengthening and interphalangeal joint arthroplasty to treat claw hammer toe. Methods A small incision was used to prolong the extensor tendons in 96 patients with severe claw toe of 177 digits, and to loosen the joint capsule of the metatarsal joint. Grinding and removing the proximal end of the articular surface of the proximal interphalangeal joints and removing the right amount of bone to make the joint take shape, the technique is corrected, the gauze bandage is bandaged, and the toe extractor is fixed externally. All patients were followed up for 1-5 years, and the evaluation standard of hammer toe was used before and after the operation. Results In the treatment of 96 cases with 177 toes, claw-toe-toe correction satisfaction, the excellent rate reached 94.35%. Conclusion The effect of small incision with extensor tendon lengthening and interphalangeal joint arthroplasty is reliable for the correction of severe claw toe.
