PPH联合小切口外痔剥除术治疗环状混合痔的疗效观察

PPH Combined with Mini-incision External Dissection in the Treatment of Circumferential Mixed Hemorrhoids:Effect Observation

为探讨吻合器痔上黏膜环切术(PPH)联合小切口外痔剥除术治疗环状混合痔的临床疗效,将120例混合痔患者随机分为观察组和对照组,各60例,观察组行PPH联合小切口外痔剥除术治疗,对照组行常规外剥内扎术治疗,对比2组手术总体疗效、术中出血量、手术时间、术后疼痛、肛门狭窄情况。结果显示,2组总有效率比较差异无统计学意义,P>0.05;但是在术中出血量、手术时间、术后疼痛程度及肛门狭窄发生率方面,观察组明显优于对照组,P<0.05。结果表明,与外剥内扎术相比,PPH联合小切口外痔剥除术治疗环状混合痔的总体疗效与外剥内扎术相当,但在术中出血量、手术时间、术后疼痛、肛门狭窄等方面具有明显优势,值得临床推广应用。

This study was to explore the clinical effect of PPH combined with mini-incision external dissec tion in the treatment of circumferential mixed hemorrhoids, randomly divided 120 cases of this hemorrhoids into observation group and control one,60 cases for each,which the former received above combined surgery, meanwhile the latter still adopted external dissection ＆. internal ligation;then compared whole effect, intraoperative bleeding volume, operative time, and the status on postoperative pain and on anal stenosis between the two groups. As results, in total effective rate there was no statistical difference between the two groups（ P 〉0.05） ,but in intraoperative bleeding volume,operative time,postoperative pain degree and anal stenosis the former was all significantly superior to the latter（ P 〈0.05）.Resuhs show that PPH corn bined mini-incision external dissection has the same whole effect as external dissection ＆ internal ligation, but in other indexes is superior to latter one,significantly.
