四黄水蜜与喜辽妥预防静脉滴注七叶皂苷钠所致静脉炎的效果对比

Comparison of Effects of External Application of Sihuang Honey Solution and Hirudoid Cream on Preventing Phlebitis Induced by Intravenous Injection of Aescine Sodium

【目的】比较外敷四黄水蜜与喜辽妥预防静脉滴注β-七叶皂苷钠所致静脉炎的效果。【方法】将120例骨科术后使用一次性精密输液器静脉滴注β-七叶皂苷钠的患者随机分为观察组和对照组,每组各60例。观察组使用四黄水蜜(由大黄、黄芩、黄檗、黄连等药物组成)外敷治疗,对照组使用喜辽妥涂擦治疗,疗程7 d。比较2组患者治疗后静脉炎的发生率及疼痛程度。【结果】静脉输注3 d后,观察组静脉炎发生率低于对照组(P<0.01),输液时局部疼痛程度轻于对照组(P<0.05);静脉输注7 d后,观察组与对照组静脉炎发生率及疼痛程度比较,差异均无统计学意义(P>0.05)。【结论】在静脉滴注β-七叶皂苷钠3 d后,四黄水蜜相对于喜辽妥能更为有效地降低静脉炎的发生率并缓解相关疼痛,但其远期效果仍有待进一步观察。

Objective To compare the preventive effect of external application of Sihuang Honey Solution andHirudoid Cream on phlebitis induced by intravenous injection of β-aescine sodium. Methods One hundred andtwenty patients after orthopedics surgery were given intravenous injection of β-aescine sodium through disposableinfusion set with precise filter. The patients were randomized into observation group and control group,60 cases ineach group. The observation group was given external application of Sihuang Honey Solution（mainly composed ofRadix et Rhizoma Rhei,Radix Scutellariae,Cortex Phellodendri and Rhizoma Coptidis）,and the control groupwas given external application of Hirudoid Cream during the intravenous injection of β-aescine sodium. Thetreatment course of the two groups covered 7 days. After treatment,the incidence of phlebitis and pain grading inthe two groups were compared. Results After intravenous injection of β-aescine sodium for 3 days,the incidenceof phlebitis was lower（P〈0.01）and pain grading was milder（P〈0.05）in the observation group than those in thecontrol group. After intravenous injection of β-aescine sodium for 7 days,the differences of incidence of phlebitisand pain grading between the two groups were insignificant（P〉0.05）. Conclusion After intravenous injection ofβ-aescine sodium for 3 days,external application of Sihuang Honey Solution is more effective on decreasing theincidence of phlebitis and reducing pain grading than external application of Hirudoid Cream,but the long-termeffect is still needed to be verified.
