经鼻中低流量无湿化氧气吸入在呼吸科患者中的循证实践 被引量:12

摘要 目的循证实践我国内地呼吸科经鼻中低流量无湿化氧气吸入患者的氧疗效果。方法根据PICO原则,提出临床问题并转化成便于检索的形式,检索数据库最终纳入8篇文献,但纳入的研究多数方法学质量等级为B。根据检索结果,进行临床实践。便利抽样法选取呼吸科经鼻中低流量吸氧的30例患者为研究对象,按照吸氧顺序,使用随机数字表将其分为观察组(15例)和对照组(15例)。对照组患者采用传统湿化给氧,观察组患者采用非湿化给氧。分别于吸氧第3天及第7天对两组患者的氧疗舒适度、睡眠质量、湿化瓶细菌污染情况等进行评价。结果无湿化中低流量吸氧降低了湿化瓶污染率,节省了护理操作时间,提高了患者睡眠质量,且不影响患者的氧疗舒适度和缺氧症状的改善,与湿化中低流量吸氧比较差异均有统计学意义(均P<0.05)。结论采用循证医学的方法,为经鼻中低流量氧气吸入不需要进行湿化提供了证据支持。 Objective To explore the effect of evidence-based practice of middle-low-flow non-humidified nasal cannula oxygen therapy for patients in department of respiration.Methods According to the principle of PICO,clinical questions were proposed and also translated for easy retrieval.Eight articles were included in the database,but the methodological quality of most researches was grade B.According to the result,30 cases with middle-low-flow nasal cannula oxygen in the department of respiration were selected.Based on the sequence of oxygen inhalation,30 case were divided into observation group(n=15)and control group(n=15)by using random number table.The control group was applied with traditional humidified nasal cannula oxygen,and the observation group was applied with nonhumidified nasal cannula oxygen.The evaluation was conducted The comfort level,sleeping quality and the contamination rate of the humidification bottle were evaluated on the third day and the seventh day of oxygen therapy.ResultsCompared with humidified oxygen therapy,the middle-low-flow non-humidified nasal cannula oxygen therapy had reduced contamination rate of the humidification bottle,less nursing operation timeimproved sleeping quality without influencing comfort level and the anoxic symptom of patients(all P〈0.05).Conclusions The evidence-based method can provide evidence support for middle-low-flow non-humidified nasal cannula oxygen therapy in clinical application.
