睡眠呼吸暂停与心力衰竭相关性及治疗手段的研究进展 被引量:1

摘要 睡眠呼吸暂停主要可分为两型,它们与心力衰竭均有显著的相关性。阻塞性睡眠呼吸暂停可通过改变胸内压、兴奋交感神经、促进炎症和氧化应激导致血管内皮损伤等途径引发心力衰竭。中枢性睡眠呼吸暂停则主要由心力衰竭时呼吸信号在循环中传递时的衰减所引发的呼吸控制不稳定导致,也可造成心力衰竭恶化,同时又被认为是心力衰竭的代偿。针对睡眠呼吸暂停在心力衰竭中的发生主要存在几种治疗方法:气道持续正压通气可解除气道阻塞;自适应伺服通气维持了较低的气道内压,适用于充血性心力衰竭患者;膈神经刺激可显著改善睡眠呼吸暂停,不适感小,具有较大潜力。在治疗方面应根据心力衰竭与睡眠呼吸暂停的相互作用进行全面考虑,选择适合的治疗方案。
