胃镜在慢性萎缩性胃炎中的诊断价值和影响因素分析

Analysis of Diagnostic Value and Influencing Factors of Gastroscopy in the Examination of Chronic Atrophic Gastritis

目的 探讨胃镜在慢性萎缩性胃炎中的诊断价值以及影响因素。方法 选取我院86例慢性萎缩性胃炎患者,对比胃镜检查和病理检查的诊断结果,探讨不同包埋方式、取材数目以及切片数量对检查结果的影响。结果 单因素分析显示,活检组织的包埋方式、取材数目以及切片数目等均会对胃镜诊断慢性萎缩性胃炎造成影响(P<0.05);Logistic回归分析显示,取材数量多、活检组织纵向包埋以及切片数目多等均是影响胃镜诊断慢性萎缩性胃炎的保护因素(P<0.05)。结论 活检组织取材数量、包埋方式以及切片的数量等均是影响胃镜诊断慢性萎缩性胃炎的重要影响因素,在实际应用胃镜对慢性萎缩性胃炎进行诊断时应注意增加活检组织取材数目、应用纵向包埋、增加切片数量、提高诊断符合率。

Objective To explore the diagnostic value and influencing factors of gastroscopy in the examination of chronic atrophic gastritis.Methods 86 cases of patients with chronic atrophic gastritis treated in our hospital were selected, and the diagnostic results of gastroscopy and pathological examination were compared.The effects on the results of different embedding methods, number of materials and the number of slices were investigated.Results Single factor analysis showed that the way of embedding biopsy, the number of samples and the number of slices would influence the diagnosis of chronic atrophic gastritis by gastroscopy（P0.05）.Logistic regression analysis showed that a large number of samples, the longitudinal entrapment of biopsy tissue and a great number of slices were protective factors of gastroscopic diagnosis of chronic atrophic gastritis（P0.05）.Conclusion The number of biopsy tissue,the way of embedding and the number of slices are all important factors affecting the diagnosis of chronic atrophic gastritis by gastroscopy.When gastroscopy is applied to diagnose chronic atrophic gastritis, attention should be paid to increasing the number of biopsy tissues, embedding longitudinally, and increasing the number of slices to improve the diagnostic accuracy.
