PAC对生育妇女流产术后即刻落实高效避孕措施的影响

The Impact of PAC(Post-abortion care) on Fertile Women in the Implementation of Efficient Contraception Immediately After Abortion

目的 探讨流产后关爱(post abortion care,PAC)对生育妇女流产术后即刻落实高效避孕措施的影响。方法 选取2015年6月至2016年6月在本院进行流产手术的育龄女性813例作为本次的研究对象,按照随机数字法将其分为观察组和对照组,其中对照组含386例女性,采用常规人工流产的程序进行,观察组含427例女性,在进行人工流产后按照PAC的流程对其进行干预,对观察组和对照组女性均进行1年随访,观察和记录两组患者的相关指标数据,包括高效避孕措施的使用情况和重复流产的发生率等内容。结果 对比两组患者术后高效避孕措施的使用情况,观察组显著优于对照组,两者间差异具有统计学意义(P<0.05);对比两组患者术后1个月的短效口服避孕药(COC)的使用率,两组差异无明显差异(P>0.05),对比两组患者术后3个月、6个月和1年的短效口服避孕药的使用率,观察组均较对照组高,两组差异具有统计学意义(P<0.05);对比两组患者术后1个月和术后3个月的宫内节育器(IUD)使用率,两组差异无明显差异(P>0.05),对比两组患者术后6个月和术后1年的IUD使用率,观察组均较对照组要高,两组差异具有统计学意义(P<0.05);对比两组患者的重复流产发生率,观察组显著低于对照组,差异具有统计学意义(P<0.05)。结论 PAC对生育妇女流产术后即刻落实高效避孕措施有明显影响,可有效改善育龄女性流产术后避孕措施的使用情况,在提升短效口服避孕药和宫内节育器的使用率等方面均起到显著作用,在较大程度上降低了重复流产的发生率,因而可以在临床上进行应用。

Objective To investigate the impact of Post Abortion Care（PAC） on effective contraception in fertile women after abortion.Method Totally 813 women of childbearing age who underwent abortion in our hospital from June 2015 to June 2016 were selected as the study subjects.According to the method of random number, they were divided into observation group and control group.The control group contained 386 women, taking routine abortion procedures.The observation group contained 427 women, to whom the PAC＇s process applied to intervene after the artificial abortion.Both groups were followed up for one year.The relevant index data of two groups of patients were recorded, including the use of highly effective contraception and the incidence of repeated abortions.Results The observation group was significantly better than the control group in effective use of contraceptive measures, with statistically significant difference（ P0.05）.The use rate of short-acting oral contraceptives（COC） was compared between the two groups 1 month after surgery.There was no significant difference between the two groups（P0.05）.The observation group was higher than the control group in oral contraceptive use rate 3 months, 6 months and 1 year after operation, with statistically significant difference（P0.05）.Intrauterine device（IUD） usage was compared between the two groups 1 month and 3 months after operation,and there was no significant difference（P0.05）.IUD was more frequently used in the observation group 6 months and 1 year after surgery than in the control group, with statistically significant difference（P0.05）.The incidence of repeated abortion of the observation group was significantly lower than that of the control group, and the difference was statistically significant（P0.05）.Conclusion PAC has a significant impact on the effective contraception of fertile women immediately after abortion.It can effectively improve the use of contraceptive measures after abortion in women of childbearing age, and play a significant role in improving short-acting oral contraceptives and the utilization of intrauterine devices.It reduces the incidence of repeated abortion to a large extent, and therefore can be clinically applied.
