针刺治疗Ⅲ型前列腺炎的疗效观察

Observations on the Therapeutic Effect of Acupuncture on Type Ⅲ Prostatitis

目的观察针刺治疗Ⅲ型前列腺炎的临床疗效及对前列腺液(EPS)中性粒细胞弹性蛋白酶(NE)的影响。方法选择67例Ⅲ型前列腺炎患者分为A组(ⅢA型)35例和B组(ⅢB型)32例,两组均给予针刺治疗。观察两组治疗前后慢性前列腺炎症状指数(NIH-CPSI)评分、EPS中白细胞(WBC)计数及NE含量的变化情况,并比较两组临床疗效。结果 A组总有效率为91.4%,B组为90.6%,两组比较差异无统计学意义(P>0.05)。两组治疗后NIH-CPSI评分、EPS中WBC计数及NE含量与同组治疗前比较,差异均具有统计学意义(P<0.01)。EPS中NE含量与NIH-CPSI评分呈正相关(P<0.01),与EPS中WBC计数呈正相关(P<0.01)。结论针刺是一种治疗Ⅲ型前列腺炎的有效方法,能降低EPS中NE含量。EPS中NE含量可以作为评价Ⅲ型前列腺炎症状严重程度的参考指标,同时对Ⅲ型前列腺炎的分型具有参考价值。

Objective To investigate the clinical efficacy of acupuncture in treating type Ⅲ prostatitis and its effect on neutrophil elastase（NE） in expressed prostatic secretions（EPS）. Methods Sixty-seven patients with type Ⅲ prostatitis were enrolled and divided into group A of 35 cases（type Ⅲ A） and group B of 32 cases（type Ⅲ B）. Both groups were treated with acupuncture. The National Institutes of Health Chronic Prostatitis Symptom Index（NIH-CPSI） score was recorded, EPS white blood cells（WBC） were counted and EPS NE content was determined in the two groups before and after treatment. The clinical therapeutic effects were compared between the two groups. Results The total efficacy rate was 91.4% in group A and 90.6% in group B; there was no statistically significant difference between the two groups（P0.05）. There were statistically significant pre-/post-treatment differences in the NIH-CPSI score and EPS WBC count and NE content in the two groups（P0.01）. EPS NE content had a positive correlation with the NIH-CPSI score（P0.01） and EPS WBC count（P0.01）. Conclusions Acupuncture is an effective way to treat type Ⅲ prostatitis and can decrease EPS NE content. EPS NE content can be used as a reference indicator to assess the severity of type Ⅲ prostatitis symptoms and has reference value to classification of Ⅲ prostatitis.
