摘要
目的观察脱细胞真皮基质填塞与传统切开挂线术治疗高位肛瘘的临床疗效。方法将97例高位肛瘘患者随机分为观察组(48例)与对照组(49例),对照组采用传统切开挂线术治疗,观察组采用脱细胞真皮基质填塞治疗。比较2组总有效率、创面愈合时间、术后并发症发生率、不良反应发生情况及术后复发率,记录2组治疗前后肛门功能积分。结果观察组总有效率明显高于对照组(P<0.05),肛门功能积分、创面愈合时间、术后并发症发生率、术后复发率均明显低于或短于对照组(P均<0.05)。2组治疗过程中均未出现严重不良反应。结论与传统切开挂线术相比,脱细胞真皮基质填塞治疗高位肛瘘疗效显著,可有效缩短创面愈合时间,保护肛门功能,减少术后并发症,防止疾病复发,安全可靠,值得临床推广应用。
Objective It is to observe the clinical effect of acellular dermal matrix packing and traditional Incision and thread-ligating therapy for the treatment of high anal fistula. Methods Ninety-seven patients with high anal fistula were randomly divided into observation group(48 cases) and control group(49 cases). The control group was treated with traditional incision and thread-ligating therapy,and the observation group was treated with acellular dermal matrix. The total effective rate,wound healing time,incidence of postoperative complications,occurrence of adverse reactions and postoperative recurrence rate were compared between the two groups,and the anal function scores before and after treatment were recorded. Results The total effective rate in the observation group was significantly higher than that in the control group( P 〈0. 05),and the anal function score,wound healing time,postoperative complication rate,and postoperative recurrence rate were significantly lower than those in the control group( P 〈 0. 05). No serious adverse reactions occurred during the treatment of both groups. Conclusion Compared with the traditional incision and thread-ligating therapy,acellular dermal matrix packing is more effective in treating high anal fistula,which can effectively shorten wound healing time,protect anal function,reduce postoperative complications,prevent disease recurrence,and is safe and reliable. It is worthy of clinical application.
