深度水解蛋白配方奶在极低出生体重早产儿中的临床应用研究 被引量:3

摘要 目的探讨深度水解蛋白配方奶在极低出生体重早产儿中的应用。方法选取潍坊医学院附属医院新生儿科2015年7月~2017年7月收治的极低出生体重早产儿92例,随机分为两组,普通早产儿配方奶喂养的46例患儿为对照组,深度水解蛋白配方奶喂养的46例患儿为观察组,对两组喂养不耐受发生率、达全肠道喂养时间、住院天数、28d时生长发育指标加以对比。结果观察组喂养不耐受的发生率(21.74%)明显低于对照组(45.65%),差异具有统计学意义(P<0.05)。达全肠道喂养时间[(13.3±2.7)d,(20.1±2.0)d]及平均住院天数[(31.6±3.4)d,(42.7±4.5)d]短于对照组,差异具有统计学意义(P<0.05)。两组早产儿的28d时体重、头围、身长比较,差异无统计学意义(P>0.05)。结论极低出生体重早产儿在住院期间采用深度水解蛋白配方奶进行喂养,能明显降低喂养不耐受发生率,缩短达全肠道喂养时间及住院时间,短期喂养并不会对患儿生长发育产生明显的影响。 Objective To explore the application of extensively hydrolyzed formula in extremely low birth weight infants. Methods From July 2015 to July 2017,92 VLBW infants admitted to our hospital were randomly assigned into two groups: the 46 infants in observation group were fed with extensively hydrolyzed formula; the 46 infants in control group were fed with general preterm infant formula. Prospective study was conducted between two groups. The feeding intolerance,the time required for total enteral feeding,average hospital stay,and physical growth on the 28 th day were compared. Results The incidence rate of feeding intolerance in observation group( 21. 74%) was statistically significantly lower than that in control group( 45. 65%). The time required for total enteral feeding[( 13. 3 ± 2. 7) d,( 20. 1 ± 2.0) d]and average hospital stay duration[( 31. 6 ± 3. 4) d,( 42. 7 ± 4. 5) d]was shorter in hydrolyzed formula group( P〈0. 05). There was no statistically significant difference in body weight,head circumference and body length on the 28 th day( P〉0. 05). Conclusions Extensively hydrolyzed formula feeding can significantly reduce the incidence rate of feeding intolerance,discharge,reduce the time required for total enteral feeding and hospital stay duration during hospitalization; it does not affect physical growth when used for a short time.
