腹腔镜胆总管探查术治疗复杂性肝胆管结石患者的临床效果

Clinical effect of laparoscopic common bile duct exploration in patients with complicated hepatolithiasis

目的探讨腹腔镜胆总管探查术治疗复杂性肝胆管结石的临床效果。方法将我院2016年9月至2017年9月收治的158例复杂性肝胆管结石患者作为研究对象,通过随机分组方式将患者划分为对照组和观察组,每组79例。对照组患者进行传统开放式胆总管探查术治疗,观察组患者进行腹腔镜胆总管探查术治疗,观察比较两组患者临床治疗效果。结果观察组患者术中出血量、手术时间、胃肠道恢复时间、住院时间均优于对照组患者(P<0.05);术后观察组患者胆红素显著低于对照组患者(P<0.05);观察组患者社会活动、生活精力、躯体活动、睡眠情况、焦虑抑郁情况、饮食情况等生活质量评分均高于对照组患者(P<0.05)。结论腹腔镜胆总管探查术治疗复杂性肝胆管结石手术创伤小、恢复快,具有应用价值。

Objective To investigate the clinical effect of laparoscop ic common bile duct exploration in patients with complicated hepatolithiasis. Methods One hundred and fifty-eight patients with complicated hepatolithiasis treated in our hospital from September 2016 to September 2017 were selected as the clinical objects. All the patients were divided into control group and observation group according to random grouping method, with 79 cases in each group. The control group of patients treated with traditional open cholecystectomy, and the observation group of patients underwent laparoscopic common bile duct exploration. The clinical efficacy of the two groups were observed. Results The intraoperative blood loss,operation time, gastrointestinal recovery time and hospital stay in the observation group were better than those in the control group（P〈0.05）. The levels of preoperative bilirubin in the observation group were significantly lower than those in the control group（P〈0.05）. The scores of social activities, life energy, physical activity, sleep quality, anxiety and depression and diet situation in the observation group were higher than those in the control group（P〈0.05）. Conclusion Laparoscopic common bile duct exploration in the treatment of complex hepatolithiasis operation trauma and rapid recovery, and therefore has clinical application value.
