隐匿性晶状体半脱位继发青光眼的临床特点及术前散瞳检查的效果

Clinical characteristics of occult lens semi-dislocation secondary glaucoma and the effect of preoperative mydriasis examination

目的探讨隐匿性晶状体半脱位继发青光眼的临床特点及术前散瞳检查的效果。方法回顾分析我院收治的20例(23眼)隐匿性晶状体半脱位继发青光眼患者的基本资料。全部患者给予散瞳检查,依据患者实际情况选择相应的手术方案,对其术后情况进行分析。结果以视力降低、头痛、眼部胀痛剧烈、恶心呕吐、虹膜震颤、散瞳后前房加深、双眼前房不对称等为主要临床特点;散瞳检查前、后患眼的前房深度有差异显著(P<0.05);散瞳检查显示22眼存在晶状体半脱位现象,检出率达到95.65%;术后患者眼压、视力与术前相比均明显改善(P<0.05)。结论隐匿性晶状体半脱位继发青光眼患者应注意与其他眼科疾病鉴别诊断,散瞳检查便于提高检出率,为疾病治疗提供相关的参考资料。

Objective To explore the clinical characteristics of occult lens semi-dislocation secondary glaucoma and the effect of preoperative mydriasis examination. Methods The basic data of 20 patients（23 eyes） with occult lens semidislocation secondary glaucoma treated in our hospital were retrospectively analyzed. All patients were given mydriasis examination and the corresponding surgical plan according to the actual situation of patients. The postoperative status of patients was analyzed. Results The main clinical features were visual acuity, headache, severe ocular distending pain,nausea and vomiting, iris tremor, deepening of anterior chamber after mydriasis, asymmetry of double anterior chamber.There was significant difference in anterior chamber depth before and after mydriasis examination（P〈0.05）. Mydriasis examination showed that 22 eyes had subluxation of lens, and the detection rate was 95.65%. Compared with before operation, the intraocular pressure and vision significantly improved after operation（P〈0.05）. Conclusion The patients with occult lens subluxation secondary glaucoma should pay attention to the differential diagnosis of other ophthalmic diseases. The mydriasis examination is convenient to improve the detection rate and provide reference materials for treatment.
