湖南西部乡村医生执业能力和职业需求探讨

A Study of Rural Doctors' Practicing Ability and Occupational Demands in Western Hunan

目的:了解湖南西部在岗乡村医生执业能力和职业需求,为提高乡村医生执业能力,规范乡村医生执业提供依据。方法:针对湖南西部的怀化、吉首两地152个乡村卫生院(室)的428名乡村医生以执业能力为主线问卷调查,并对年龄、职称结构、从业年限、职业需求及职业精神等进行相应探讨。结果:湖南西部在岗乡村医生中专占61.7%,大专占13.8%,本科占0.7%,无医学学历背景的占23.8%;获执业(助理)医师证占27.4%,乡村医生证占72.6%;年龄大于60岁的占17.6%;从医11~30年的占63.3%;主治医师只占2.5%,无医生职称的占23.8%;对临床技能、常见病多发病诊断处理、基本药物制度及合理用药等知识的需求在90%以上;职业精神村医优于乡医。结论:湖南西部在岗乡村医生执业能力离国家要求的距离较大,对临床基本医疗技能需求较迫切,建议政府加大乡村医生培养力度,提高乡村医生的执业能力。

Objective To understand the practice ability and professional needs of rural doctors working in the west of Hunan,and to provide evidence for improving the ability of rural doctors to practice and standardize the practice of rural doctors.Method To investigate 428 rural doctors from 152 rural health centers in two cities in Huaihua and Jishou in the west of Hunan.Taking the practice ability as the main line,the questionnaire survey was carried out,and the age,the title structure,the number of years of employment,the professional needs and the spirit of the profession were discussed.Results In the western rural doctors of Hunan province,secondary technical education accounted for 61.7%,college education accounted for 13.8%,undergraduate education accounted for 0.7%,and no medical background accounted for 23.8%.27.4% of doctors were certified doctors,and 72.6%were rural doctors.Accounted for 17.6% of the age of 60,doctors engaged in 11 to 30 years accounted for 63.3%,primary care physician only accounted for 2.5%,23.8% without medical certification.More than 90% of the doctors,clinical skills,diagnosis and treatment of common diseases,basic drug system and rational use of drugs are needed.Professional spirit is better than rural medicine.Conclusion In the west of Hunan province,the ability of practicing doctors in rural areas is greatly different from the demand of the country,and the demand for clinical basic medical skills is urgent.It suggested that the government should strengthen the training of rural doctors and improve their practicing ability.
