贵州省凯里市苗族原发性高血压与中医体质相关性研究

Relationship Between Hypertension and TCM Constitution of Miao Nationality in Kaili

目的：探讨凯里苗族人群高血压病患者的体质特点及高血压病发病的体质因素,从而更好地指导临床制定个体化的中医预防保健干预措施,为凯里苗族高血压病的中医药防治提供有效依据。方法：根据王琦将体质分为9类的理论,平和质为健康体质,其余8种为偏颇体质,各种偏颇体质均有其相应的易患疾病。采用问卷调查的方式,对凯里543例苗族人群进行调查,严格按照中医体质量表及《中医体质分类与判定》标准,进行中医体质辨识,将苗族人群分为高血压和非高血压两组进行对照分析。采用多元logistic回归分析筛选高血压有意义的主要体质影响因素。结果：控制性别、身高、体质量、饮食喜好、年龄、婚姻状况、职业等因素后,运用多元logistic回归分析模型,影响凯里市苗族原发性高血压的体质因素主要有气虚质、阳虚倾向、阴虚质、湿热质、痰湿质。结论：经过对凯里市苗族原发性高血压和非高血压人群的调查,找出性别、有无高血压家族史、有无药物过敏史、婚姻状况、饮食喜好与高血压的关系。了解了苗族高血压患者的中医体质分布特点,找出了苗族原发性高血压与中医体质的相关性,对指导临床制定个体化的中医预防保健和干预治疗措施具有积极意义。

Objective：To investigate the physical characteristics in patients with hypertension in Kaili Miao population so as to better guide clinical individualized TCM preventive health interventions for the prevention and treatment of hypertension in the Miao Nationality in Kaili. Methods：According to WANG Qi＇s theory of the nine constitutions,the nature constitution is the health constitution,and the other 8 kinds are biased constitutions. All kinds of biased constitution have the corresponding diseases. By means of questionnaire survey on Kaili 543 people,strictly accordance with the TCM scale and The Chinese Constitution Classification and Decision Standards of TCM Constitution identification,the Miao nationality is divided into hypertension and non hypertension group. Multivariate logistic regression analysis was used to screen the main influential factors of hypertension. Results：After the control of sex,height and weight,diet,age,marital status,occupation and other factors,using multivariate logistic regression analysis model,the physical factors affecting the Kaili Miao primary hypertension are mainly Qi deficiency and Yang deficiency,Yin deficiency,damp-heat,phlegm-dampness. Conclusion：Through the investigation of the population with hypertension and non hypertension in Miao Nationality in Kaili,the relationship among gender,family history of hypertension,history of drug allergy,marital status,dietary preferences and hypertension has been found out. To understand the distribution characteristics of TCM constitutions of Miao Nationality in hypertension patients,we find out the correlation between Miao primary hypertension and TCM constitutions,which has positive significance to guide the clinical treatment of individualized health prevention and intervention of traditional Chinese medicine.
