基于聚类分析研究林国华教授治疗带状疱疹的选穴规律

Regulation of acupoint of Professional Lin Guo-hua in treating herpes zoster based on hierarchical cluster analysis

目的研究林国华教授治疗带状疱疹的选穴规律。方法回顾性分析2016年1月至2017年6月广州中医药大学第一附属医院针灸科收治的124例符合带状疱疹诊断的患者病历资料,运用数据挖掘技术进行筛选,观察林国华教授治疗带状疱疹中的穴位使用频数与频率。通过分层样品聚类分析,采用Ward法进行聚类,以平方欧式距离(SED)作为度量,以凝聚过程反应表达聚类分析过程,以系统树图表示分析结果。结果 124例病历资料中针灸处方共620份。其中,使用频数排名前20位的穴位中,足少阳胆经穴位使用频率最高,达到25%(5/20),而最常用的穴位是夹脊穴,达到98.06%(608/620)。以SED为度,聚类分析可分为聚四类(SEP=4)、聚三类(SEP=5)与聚二类(5<SEP<10),以聚三类结果最符合选穴规律情况。结论林国华教授治疗带状疱疹的用穴规律主要可以聚成三类,主要以"辨病、辨经和辨证"三维取穴为基础。

Objective To study the regulation of acupoint of Professional Lin Guo-hua in treating herpes zoster. Methods Clinical data of 124 patients with herpes zoster were collected from the Department of Acupuncture of First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine from January 2016 to June 2017. The frequency and rate of acupoint used by Professional Lin Guo-hua were observed. Then based on hierarchical cluster analysis, data were analyzed statistically with the Ward＇s linkage with the measurement of squared Euclidean distance（SED）,table of the agglomeration schedule and the dendrogram using to show results. Results Among the 124 cases, 620 acupuncture and moxibustion prescriptions were collected. Moreover, the Gallbladder meridian was used much more frequent than the others, with the frequency of 25%（5/20）, and the most frequently used acupoint was Jiaji points with98.06%（607/620）. With the measurement of SED, the data could be clustered into 4（SEP=4）, 3（SEP=5）, and 2（5SEP10） categories, with the 3 categories best conforming to the regulation of acupoint. Conclusion The regulation of Professional Lin Guo-hua in treating the herpes zoster could be clustered to 3 categories, which was based on specific methodology related to＂distinguish the disease, meridian and syndrome＂.
