甘油果糖联合依达拉奉治疗高血压性脑出血的效果及对神经功能恢复和肾功能损害的影响分析 被引量:5

摘要 目的:探究甘油果糖联合依达拉奉(EDA)治疗高血压性脑出血(HIH)疗效及对神经功能恢复和肾功能损害的影响。方法:选取笔者所在医院124例HIH患者为研究对象,采用随机数字表法分为EDA组(对照组,n=62)和EDA联合甘油果糖组(观察组,n=62)。两组均给予降压、吸氧、保护神经等常规疗法,对照组给予EDA和甘露醇治疗,观察组则予以EDA联合甘露醇、甘油果糖交替治疗。比较两组治疗前与治疗15 d后血肿体积、水肿体积、神经功能[改良爱丁堡-斯堪的那维亚神经功能缺损评分量表(MESSS)]、日常活动能力(ADL)[改良Barthel指数(BMI)]、肾功能[血清胱抑素C(Cys-C)、血尿素氮(BUN)]变化情况差异。结果:治疗15 d后,两组血肿及水肿体积和MESSS评分较治疗前降低,BMI评分则升高,差异有统计学意义(P<0.05),且观察组变化幅度大于对照组,差异有统计学意义(P<0.05);两组Cys-C、BUN水平均较治疗前升高(P<0.05),且对照组高于观察组,差异有统计学意义(P<0.05)。结论:EDA联合甘油果糖及甘露醇疗法对HIH患者治疗效果较好,且对肾功能损害较小,可促进患者血肿、水肿体积缩小,利于患者神经功能和日常活动能力恢复。 Objective:To explore the effect of glycerin fructose combined with edaravone(EDA) on hypertensive intracerebral hemorrhage(HIH) and its influence on the recovery of neurological function and impairment of renal function.Method:124 HIH patients treated in our hospital were taken as the research objects,and they were randomly divided into EDA group(control group,n=62) and EDA combined with glycerin fructose group(observation group,n=62).Both groups were given antihypertension,oxygen inhalation,nerve protection and other conventional therapy,while the control group was given additional EDA and mannitol treatment,and the observation group was given additional alternative treatment of EDA combined with mannitol or glycerin fructose.The differences of hematoma volume,edema volume,neurological function [modified Edinburgh Scandinavia Stroke Scale(MESSS)],activities of daily life(ADL)[modified Barthel index] and renal function[serum cystatin C(Cys-C),blood urea nitrogen(BUN)] before and 15 d after treatment between the two groups were compared.Result:15 d after treatment,the hematoma volume,edema volume and MESSS scores in the two groups were all significantly decreased,while the BMI scores in the two groups were obviously increased(P〈0.05),and the change ranges in the observation group were greater than those in the control group(P〈0.05).The Cys-C and BUN levels in the two groups were all significantly increased(P〈0.05),and the Cys-C and BUN levels in the control group were obviously higher than those in the observation group(P〈0.05).Conclusion:The alternative treatment of EDA combined with glycerin fructose or mannitol has better effects in the treatment of patients with HIH,and less damage to renal function,can promote the reduction of hematoma and edema volume,and it is also conducive to the recovery of neurological function and ADL.
