鼻内镜和普通开窗刮治术治疗上颌骨囊肿的疗效比较

Comparison of curative effects of endoscopic sinus surgery and conventional fenestration for maxillary cyst

目的:观察鼻内镜和普通开窗刮治术治疗上颌骨囊肿的疗效。方法:选取100例上颌骨囊肿患者为研究对象,按治疗方案的不同分为观察组和对照组,各50例。观察组患者鼻内镜下行上颌骨囊肿开放术,对照组实施普通开窗刮治术,比较两组患者术后并发症发生率和囊肿复发率。结果:观察组患者术后并发症发生率为4.00%,低于对照组的14.00%(χ~2=6.105,P=0.013);随访期间,观察组患者无复发病例,对照组患者复发率为8.00%(4/50),组间比较,差异有统计学意义(χ~2=8.333,P=0.004)。结论:与普通开窗刮治术相比,应用鼻内镜下上颌骨囊肿开放术治疗上颌骨囊肿,并发症发生率和复发率较低,对提高手术治疗效果、改善预后具有重要临床意义。

Objective： To investigate the curative effects of endoscopic sinus surgery and conventional fenestration in the treatment of maxillary cyst. Methods： 100 cases of maxillary cyst were divided into the observation group and the control group,50 cases in each group. The observation group received nasal endoscopic maxillary cyst opening,while the control group underwent routine fenestration curettage,and the incidence of postoperative complications and the recurrence of cysts during the follow-up period were compared between the two groups. Results： The incidence of postoperative complications in the observation group was 4. 00 %,which was significantly lower than that of 14. 00 % in the control group（ χ^2= 6.105、P = 0. 013）. During the follow-up period,no cyst recurrence occurred in the observation group. In the control group,4 patients had cyst recurrence,accounting for about 8. 00 %（ χ^2= 8. 333、P = 0. 004）. Conclusion： Compared with conventional fenestration,the open surgery under nasal endoscope for the treatment of cyst of maxilla is better,which can effectively reduce the incidence of postoperative complications and prevent the recurrence of the disease,improving the prognosis of patients,with clinical significance and application value.
