超声心动图技术在慢性肝病中的应用及研究进展

Application and Research Progress of Echocardiography in Chronic Liver Diseases

慢性肝病以肝脏病变为基础,逐渐影响全身各个系统的功能,并可导致心脏血管功能变化,早已发现肝硬化患者存在静息性心动过速、肢体远端温暖、脉压差增大、毛细血管搏动征等高血流动力循环的表现。慢性肝病的心血管主要并发症为:(1)肝硬化性心肌病;(2)肝肺综合征;(3)门脉性肺动脉高压。超声心动图是评估慢性肝病患者心血管功能障碍的重要筛查方法。通过评估心脏收缩及舒张功能指标,评价肺内血管分流情况,估测肺动脉压力和右心功能改变等作出有效诊断,且由于超声心动图非侵入性、实时快速成像和诊断准确率高等优点成为首选检查方法,为进一步诊治指明方向。

Chronic liver disease is based on liver disease,gradually affects the function of the whole body system,and can lead to changes in cardiac vascular function. Patients with cirrhosis have been found to have resting tachycardia, distal limb warmth, increased pulse pressure difference, and the capillaries beating sign is positive for the performance of a high blood flow regimen. The main cardiovascular complications of chronic liver disease are：（1） cirrhotic cardiomyopathy;（2） hepatopulmonary syndrome ;（3）portal hypertension. Echocardiography is an important screening method for assessing cardiovascular dysfunction in patients with chronic liver disease. By evaluating systolic and diastolic function indicators, evaluating pulmonary vascular shunt conditions, estimating pulmonary right heart function changes to make an effective diagnosis. Echocardiography has become the preferred method of its non-invasive, real-time rapid imaging and high diagnostic accuracy, and it indicates the direction for further diagnosis and treatment.
