急性ST段抬高型心肌梗死患者PCI术后心房颤动发生的预测因素 被引量:9

摘要 目的探讨急性ST段抬高型心肌梗死(STEMI)患者冠状动脉介入治疗(PCI)术后心房颤动(房颤)发生的预测因素。方法选取STEMI急诊PCI术后1周内初发房颤患者24例(房颤组),另选取同期STEMI急诊PCI术后1周内未发生房颤患者15例(无房颤组),PCI术后24 h内,检测血清B型脑钠尿肽(BNP)、钾离子(K^+)浓度及动脉血酸碱度(pH)、平均收缩压(SBP)、平均舒张压(DBP)、平均心率(HR)、计算平均动脉压/心率指数(MAP/HR)、最大P波时限(Pmax)、最小P波时限(Pmin)、P波离散度(Pd)、平均外周血氧饱和度(SpO_2)。二元Logistic回归检测相关因素的预测价值。结果房颤组患者在Pmin、SBP、DBP、K^+浓度、pH较无房颤组差异无统计学意义(P均>0.05),血清BNP、Pmax、Pd升高,HR增加,SpO_2及MAP/HR降低(P均<0.05)。各因素对房颤发生总判对率为92.3%。结论综合BNP、MAP/HR、HR、Pmax、Pd、SpO_2因素能有效预测STEMI患者PCI术后房颤的发生。
