前臂肌肉与真皮下自体移植在SHPT患者甲状旁腺全切术中的应用比较 被引量:4

摘要 目的比较前臂肌肉与真皮下自体移植方案在继发性甲状旁腺功能亢进(SHPT)患者甲状旁腺全切术中的应用效果。方法选取SHPT患者100例,随机分为A组、B组各50例。在甲状旁腺全切术中,A组、B组分别采用前臂肌肉、真皮下自体移植方案进行甲状旁腺移植。比较两组移植手术时间、术中出血量、住院时间及术后血磷、血钙、免疫反应性甲状旁腺激素(i PTH)水平、复发率等。结果 B组移植手术时间短于A组(P<0.05),两组术中出血量、住院时间比较差异无统计学意义(P均>0.05)。B组术后1周时i PTH水平低于A组(P<0.05)。A、B组术后复发例数分别为3、1例,两组术后复发率比较差异无统计学意义(P>0.05)。结论在SHPT患者甲状旁腺全切术中,与前臂肌肉自体移植方案相比,应用前臂真皮下自体移植方案可有效缩短移植手术用时,促进术后早期i PTH水平降低。
