3D-Slicer虚拟现实技术在神经外科脑肿瘤术前计划的应用 被引量:17

摘要 目的探讨3D-Slicer软件在神经外科脑肿瘤术前计划的应用价值。方法运用3D-Slicer软件以DICOM格式导入20例脑肿瘤病人的影像资料并进行三维重建,获得可视化虚拟现实图像,了解病变部位及其相应功能结构的三维位置关系,使用软件内相关模块测量三维重建的各项参数,制定详细的手术入路,术前模拟手术操作。结果全部病例按最佳的模拟手术方案进行手术,均顺利完成手术,显微镜及内镜下全切除肿瘤。术后3个月复查MRI未见肿瘤复发。病人术后未出现新发症状,无死亡病例。结论运用3D-Slicer软件进行术前虚拟重建并制定手术计划,可取得大量立体解剖信息,以此指导术者设计手术切口、选择手术入路以及切除肿瘤,可提高手术成功率,并在脑功能保护、减少术后并发症方面发挥作用。 Objective To explore the application value of 3D-Slicer software in preoperative plan for brain tumor. Methods Imaging data of 20 patients with brain tumors were imported into DICOM format and reconstructed through 3D-Slicer software to get visually virtual reality image for understanding the 3D positional relationship of lesion and the corresponding functional structure. The parameters of 3D reconstruction were measured by software related modules, then, a detailed surgical approach was established, and operative procedures were simulated before the operation. Results All the patients were operated according to the best simulated operation plan, and all underwent the operation successfully. Total tumor resection under microscope and endoscope were achieved in all the patients. MRI showed no tumor recurrence 3 months after the operation. No new symptoms occurred after the operation and no patient died. Conclusions A lot of 3D anatomic information can be got from virtual reconstruction and surgical planning through 3D-Slicer software before the operation, which can guide the surgeon to design surgical incision, select surgical approach and resect the tumor. Otherwise, it can improve the success rate of surgery, and play a role in protecting the brain function and reducing postoperative complications.
