宫腔镜手术治疗剖宫产术后子宫瘢痕妊娠患者的疗效观察

Observation on efficacy of hysteroscopic surgery in the treatment of patients with cicatricial uterus pregnancy after cesarean section

目的探究宫腔镜手术治疗剖宫产术后子宫瘢痕妊娠(GSP)患者的临床疗效。方法 148例剖宫产术后子宫瘢痕妊娠患者,采用随机数字法分为对照组和观察组,各72例。对照组采用传统清宫术治疗,观察组采用宫腔镜手术治疗。比较两组患者临床治疗疗效以及手术时间、住院时间、手术流血量。结果观察组患者手术时间为(25.81±1.41)min、住院时间为(5.13±1.13)d、手术流血量为(65.62±6.21)ml,均优于对照组的(40.39±2.77)min、(7.78±3.13)d、(106.61±12.59)ml,差异有统计学意义(P<0.05)。观察组患者治疗总有效率为97.30%,高于对照组的89.19%,差异有统计学意义(P<0.05)。结论宫腔镜手术治疗剖宫产术后子宫瘢痕妊娠,手术安全、有效,患者术后恢复快,因此具有一定的临床推广应用价值。

Objective To investigate the clinical efficacy of hysteroscopic surgery in the treatment of patients with cicatricial uterus pregnancy（GSP） after cesarean section. Methods A total of 148 patients with cicatricial uterus pregnancy after cesarean section were divided by random number table method into control group and observation group, with 72 cases in each group. The control group was treated with traditional complete curettage of uterine cavity, and the observation group was treated with hysteroscopic surgery. Comparison were made on clinical treatment efficacy, operation time, hospitalization time and surgical bleeding volume between the two groups. Results The observation group had better operation time as（25.81±1.41）min, hospitalization time as（5.13±1.13） d and surgical bleeding volume as（65.62±6.21） ml than（40.39±2.77） min,（7.78±3.13） d and（106.61±12.59） ml in the control group, and their difference was statistically significant（P〈0.05）. The observation group had higher total treatment effective rate as 97.30% than 89.19% in the control group, and the difference was statistically significant（P〈0.05）. Conclusion Hysteroscopic surgery is safe and effective for cicatricial uterus pregnancy after cesarean section with quick recover after operation. So it contains a certain clinical promotion and application value.
