血浆纤维蛋白原在小儿阑尾炎患者中的变化及临床意义

Changes of plasma fibrinogen in children with appendicitis and its clinical significance

目的探讨血浆纤维蛋白原(Fib)在小儿阑尾炎患者中的变化及临床意义。方法回顾性分析我院进行阑尾炎手术病例,病理证实为阑尾炎的患者852例,按其病理分型分单纯组386例,化脓组381例,坏疽组85例,全部采用自动血凝仪检测血浆中Fib含量。结果化脓组、坏疽组的Fib、WBC显著高于单纯组,差异有统计学意义(P<0.01),而坏疽组的Fib、WBC又较化脓组高,差异有统计学意义(P<0.01),Fib与WBC相关性呈正相关(r=0.261,P<0.01)。结论在小儿阑尾炎中Fib、WBC随着病理分型而变化,病理越重Fib、WBC越高,Fib、WBC对判断阑尾炎的轻重以及预分型有一定帮助,可以为临床决策提供更多依据。

Objective To investigate the changes and clinical significance of plasma fibrinogen（Fib） in children with appendicitis. Methods Retrospective analysis was made on the clinical data of 852 cases with the appendicitis who were divided into three group according to the pathological classification. The three groups were simple control group（386 cases）,the suppuration group(381 cases） and gangrene group(85 cases）. All plasma levels of Fib were measured by automatic hemagglutination instrument. Results The levels of Fib and WBC in the suppuration group and gangrene group were significantly higher than those in simple control group,difference was statistically significant(P〈0. 01）. And levels of Fib and WBC in gangrene group were higher than those in suppuration group,difference was statistically significant（P〈0. 01）. In addition,Fib and WBC were positively correlated. Conclusion The levels of Fib and WBC changes with the pathological classification in pediatric appendicitis. The more serious pathological classification means the higher level of Fib and WBC. As a consequence,it can diagnose the severity and preassigned type of appendicitis. It can also provide more evidence for clinical decision making.
