紫银荷冰汤治疗慢性扁桃体炎扁桃体切除术后患者咽部疼痛的临床研究

Clinical Research on the Treatment of Pharyngeal Pain Following Tonsillectomy of the Chronic Tonsillitis Patients Using Ziyin Hebing Decoction

目的观察紫银荷冰汤对扁桃体切除术后咽部疼痛患者镇痛的疗效。方法选择2012年7月—2016年10月在湖州师范大学附属第一医院住院的慢性扁桃体炎扁桃体切除术后患者,使用区组随机分组法将患者分为对照组和观察组,每组160例。观察组使用紫银荷冰汤漱口,对照组使用冰水漱口,两组均每天含漱5次,每次含漱10~20mL,每次10min,不能耐受疼痛时口服止痛药物氨酚曲马多片1片。使用视觉模拟量表(VAS)评分评估两组术后12h和术后1、2、3、5、7天疼痛程度,并记录氨酚曲马多片服用例数和数量。结果与对照组同期比较,观察组术后2、3、5天VAS评分下降(P<0.05,P<0.01),术后2、3、5天氨酚曲马多片用量下降(P<0.05,P<0.01),术后2、3天服用氨酚曲马多片平均数量下降(P<0.05,P<0.01)。结论紫银荷冰汤可缓解扁桃体切除术后咽部疼痛患者疼痛程度,并减少止痛片用量。

Objective To investigate the analgesic effects of Ziyin Hebing Decoction（ZYHBD） on pharyngeal pain of the chronic tonsillitis patients following tonsillectomy. Method From July,2012 to October, 2016,the post-operation chronic tonsillitis inpatients in First Affiliated Hospital of Huzhou Normal University were randomly assigned to the control group and the observation group,160 cases in each group. The observation group gargled with ZYHBD and the control group gargled with ice water. Each group gargled 5 times a day, 10 min per gargle with 10-20 mL Liquid. When the pain became intolerable, the patients were given one tablet of anti-inflammatory medication,paracetamol and tramadol hydrochloride tablet,orally. The Visual Analogue Scale（VAS） score was used to evaluate the degree of pain at 12 h and 1,2,3,5,7 days after the operation. The number of given paracetamol and tramadol hydrochloride tablets and the number of the patients who took the tablets were recorded. Result Compared with the control group at the same time of the experiment,the VAS score of the observation group decreased at 2,3,5 days after the operation（P〈0.05,P〈0.01）,the number of the given paracetamol and tramadol hydrochloride tablets decreased at 2, 3, 5 days after the operation（P〈0.05,P〈0.01）,the average number of the given paracetamol and tramadol hydrochloride tablets decreased at 2, 3 days after the operation（P〈0.05, P〈0.01）. Conclusion ZYHBD can alleviate the degree of pharyngeal pain following tonsillectomy of the chronic tonsillitis patients and reduce the dosage of pain killers.
