掌侧解剖锁定钢板治疗不稳定型桡骨远端骨折的效果观察

Efficacy of palmar anatomic locking plate on unstable distal radius fractures

目的:探讨掌侧解剖锁定钢板治疗不稳定型桡骨远端骨折的效果观察。方法:将2013年6月-2017年6月在我院骨科治疗的100例不稳定型桡骨远端骨折患者随机分为两组,对照组采用传统手法复位后小夹板固定治疗,观察组采用掌侧解剖锁定钢板治疗,比较两组患者的治疗效果、腕关节功能恢复情况、并发症发生率。结果:观察组Gartland-Werley评分优良率为98%,明显高于对照组的78%(P<0.05);观察组骨折愈合时间、关节握力评分、腕关节活动范围评分、手部外观评分、功能满意度评分等与对照组相比有明显差异(P<0.05);观察组骨折移位、骨折畸形愈合、骨折不愈合等并发症发生率明显低于对照组(P<0.05)。结论:掌侧解剖锁定钢板治疗不稳定型桡骨远端骨折的效果显著,能加强对骨折的固定,加速骨折愈合,有效恢复腕关节功能,减少并发症发生率,具有积极的临床意义。

Objective： To explore the efficacy of palmar anatomic locking plate on unstable distal radius fractures. Methods： 100 patients with unstable distal radius fractures in our hospital from June 2013 to June 2017 were selected and randomly divided into two groups. The control group was treated by small splint fixation after traditional manual reduction, and the observation group was treated by palmar anatomic locking plate. The therapeutic effect, recovery of wrist joint function and the incidence of complications were compared between two groups. Results： Gartland-Werley score excellent and good rate in the observation group was 98%, which was significantly higher than 78% in the control group（P〈0.05）. The difference of fracture healing time, grip strength score, wrist movement range score, hand appearance score, functional satisfaction score, etc. in two groups was statistical significance（P〈0.05）. The incidence of fracture displacement, fracture malunion and fracture nonunion in the observation group was significantly lower than that in the control group（P〈0.05）. Conclusion： The treatment of unstable distal radius fractures with palmar anatomic locking plate was very effective. It could strengthen fixation, accelerate fracture healing, recover wrist function effectively and reduce the incidence of complications, which was of positive clinical significance.
