西药房常见处方调剂问题的分析及改善方案研究

An analysis of the problems of common prescription dispensing in dispensary for western medicine and research on improvement scheme

目的:分析西药房常见处方调剂问题,并在此基础上探讨有效的改善方案。方法:选取我院西药房2015年-2017年发生处方调剂差错的处方120份进行回顾性分析。对120份处方发生调剂差错的原因进行分析,并记录其调剂差错类型,包括药品种类、用法用量、用药禁忌差错等。在统计处方调剂差错类型,分析调剂差错原因的基础上,制定有效的改善方案并实施。结果:120份调剂差错处方中,差错类型发生率从高到低依次为同一药物规格分辨问题、药物品种数量问题、药物用法用量问题和药物禁忌问题,分别占比31.7%、28.3%、25.0%、15.0%。120份处方的调剂差错原因主要包括同品规格不同、药品名称类似、药品包装类似、药品摆放位置出错、处方错误、药品配伍禁忌出错和药品用法交代不清晰等,其中占据前三位的从高到低依次为药品配伍禁忌出错、药品摆放位置出错和药品名称类似。结论:西药房常见处方调剂问题主要包括同一药物规格分辨问题、药物品种数量问题、药物用法用量问题和药物禁忌问题,只有从提高工作人员的意识,完善规章制度,加强管理等几个全面入手,才能够降低处方调剂差错发生率,提高西药房服务质量。

Objective： To analyze the problems of common prescription dispensing in dispensary for western medicine and to discuss the effective improvement scheme. Methods： 120 prescriptions with prescription dispensing errors from 2015 to 2017 in our hospital was made a retrospective analysis. The causes of dispensing errors in 120 prescriptions were analyzed, and the types of dispensing errors were recorded, including drug types, usage and dosage, taboo errors, etc. On the basis of the statistical error types of prescription dispensing and the analysis of the reasons for the adjustment errors, effective improvement schemes were formulated and implemented. Results： In 120 dispensing error prescriptions, the incidence of error types ranged from high to low in order to distinguish the same drug specification, the number of drug varieties, drug usage and drug taboo problems, accounting for 31.7%, 28.3%, 25%, 15% respectively. The prescription dispensing error reasons mainly included the error of the same products of different specifications, the error of the name of the drug, the error of similar drug packaging and the error of similar drug and similar position error, error of prescription drug incompatibility and unclear drug usage. The top three errors were drug incompatibility, drug position error and similar drug name error. Conclusion： The pharmacy prescription dispensing problems included common specifications of the same drug problem, drug problem resolution, the number of drug usage problems and drug problems, only by improving staff awareness, perfecting rules and regulations, strengthening the management could the hospital reduce the incidence of prescription errors and improve pharmacy service quality.
