慢性病自我管理项目在肿瘤PICC化疗患者管理中的应用

Application of chronic disease self-management project in the management of patients who received cancer PICC chemotherapy

目的探讨慢性病自我管理项目在肿瘤经外周静脉置入中心导管(PICC)化疗患者管理中的应用效果。方法选取2015年6月至2017年6月本院肿瘤科收治的肿瘤PICC患者80例,随机将患者等分为观察组及对照组,对照组置管期间行常规性护理干预,观察组置管期间实施慢性病自我管理项目,比较两组患者干预前后疾病知识水平、自我效能、自护能力及并发症发生情况。结果观察组患者干预后疾病知识评分、自我效能评分、自护能力评分均高于对照组(P<0.05),静脉炎、管腔回血、导管堵塞、导管外移、导管感染等并发症发生率低于对照组(P<0.05),患者满意率高于对照组(P<0.05)。结论慢性病自我管理项目能有效提高肿瘤PICC患者疾病知识及疾病管理能力,降低置管期间并发症发生率,提高患者治疗满意率。

Objective To explore the application effect of chronic disease self - management project in the management of patients who received cancer peripherally inserted central venous catheters （PICC） chemotherapy. Methods To select a total of 80 patients with tumor PICC who were treated in the on- eology department of our hospital from June 2015 to June 2017 as the research objects and they were randomly divided into observation group and control group. The control group received routine nursing intervention during catheterization. A chronic disease self - management project was implemented in the observation group during catheterization. The level of disease knowledge, self - efficacy, self - care ability, and complications before and after intervention in both groups were compared. Results After the intervention,the scores of disease knowledge, self- efficacy and self-care ability of the patients in the ob- servation group were higher than those in the control group（P 〈 0.05 ）. The incidences of complications such as phlebitis, lunfinal blood return, catheter obstruction, outflow of catheter, and catheter infection were lower than that of the control group （ P 〈 0.05 ）. The patient satisfaction rate of the observation group was higher than that of the control group （P 〈 0.05 ）. Conclusion The chronic disease self - msnagement project can effectively improve the disease knowledge and disease management ability of oneology PICC patients, and reduce the incidence of complications during catheterization. Meanwhile, it can improve patient satisfaction with treatment.
