胎心监护与孕晚期超声检测胎儿脐动脉和大脑中动脉血流对子痫前期围生儿预后的相关研究 被引量:7

摘要 目的研究胎心监护与孕晚期超声检测胎儿脐动脉和大脑中动脉血流对子痫前期围生儿预后的相关性。方法 100例孕产妇及其胎儿,随机分为正常妊娠组与子痫前期组,各50例。两组均行胎心监护与孕晚期超声检测胎儿脐动脉和大脑中动脉血流,分析其检测结果。结果子痫前期组胎心监护图形的正常型比例低于正常妊娠组,可疑型、异常型比例均高于正常妊娠组,差异有统计学意义(P<0.05)。子痫前期组胎儿脐动脉各血流参数检测值均高于正常妊娠组,大脑中动脉各血流参数检测值均低于正常妊娠组,差异有统计学意义(P<0.05)。子痫前期组围生儿体重(2815±223)g、胎盘重量(558±53)g、Apgar评分(8.8±0.5)分低于于正常妊娠组的(3384±265)g、(695±74)g、(9.6±0.4)分,差异有统计学意义(P<0.05)。结论胎心监护与孕晚期超声检测胎儿脐动脉和大脑中动脉血流对子痫前期围生儿预后的预测与评估具有一定的指导意义。 Objective To study the correlation between fetal heart monitoring and late pregnancy ultrasound detection of fetal umbilical artery and middle cerebral artery blood flow in perinatal prognosis of preeclampsia. Methods A total of 100 parturient women and their fetus were randomly divided into normal pregnancy group and preeclampsia group, with 50 cases in each group. Both groups received fetal heart monitoring and late pregnancy ultrasound detection of fetal umbilical artery and middle cerebral artery blood flow, and their detection results was analyzed. Results The preeclampsia group had lower proportion of normal fetal heart monitoring pattern than normal pregnancy group, and higher proportion of suspected and abnormal pattern than that of normal pregnancy group. Their difference was statistically significant(P〈0.05). The preeclampsia group had higher blood flow parameter detection value in fetal umbilical artery than normal pregnancy group, and lower blood flow parameter detection value in middle cerebral artery than normal pregnancy group. Their difference was statistically significant(P〈0.05). The preeclampsia group had lower perinatal weight as(2815±223) g, placental weight as(558±53) g, Apgar score as(8.8±0.5) points than(3384±265) g,(695±74) g and(9.6±0.4) points in normal pregnancy group. Their difference was statistically significant(P〈0.05). Conclusion Fetal heart monitoring and late pregnancy ultrasound detection of fetal umbilical artery and middle cerebral artery blood flow shows certain guiding significance for prediction and evaluation of perinatal preeclampsia.
