痰热清注射液对合并COPD肺癌患者围手术期血液流变学和肺功能的影响

Effect of Tanreqing injection on hemorheology and pulmonary function during the perioperative period of COPD patients with lung cancer

目的探讨痰热清注射液对慢性阻塞性肺疾病(COPD)合并肺癌患者围手术期的疗效及对血液流变学、肺功能的影响。方法将2015年12月~2016年6月因肺癌于本院行肺叶切除术治疗的80例患者纳入研究并随机分为两组,对照组40例患者于术前7 d至术后7 d采用常规治疗,观察组40例则联合痰热清注射液静滴治疗。比较两组治疗前后超敏C反应蛋白(hs-CRP)及血液流变学指标纤维蛋白原(Fib)、D-二聚体(D-D)的改善情况,比较两组第一秒用力呼气量(FEV1)、FEV1/FVC等肺功能指标改善情况。结果治疗后观察组hs-CRP及Fib、D-D等指标低于对照组,差异具统计学意义(P<0.05);治疗后两组第一秒用力呼气量(FEV1)、FEV1/FVC等肺功能指标降低,但观察组指标仍高于对照组(P<0.05)。结论痰热清注射液在COPD合并肺癌围手术期患者中有较高的应用价值,可有效预防高凝状态,减轻肺功能损伤,值得推广。

Objective To investigate the effect of Tanreqing injection on perioperative period of chronic obstructive pulmonary disease（COPD）combined with lung cancer and its effects on hemorheology and pulmonary function. Methods From December 2015 to June 2016 for 80 cases of lung cancer in our hospital for surgery in patients were enrolled and randomly divided into two groups, control group of 40 patients from 7 d before operation to 7 d after operation with conventional treatment, the observation group of 40 cases combined with Tanreqing injection. The improvement of hypersensitivity C reactive protein（hs-CRP） and hemorheological indexes, fibrinogen（Fib） and D two dimer（D-D） in the two groups before and after treatment were compared, and the improvement of pulmonary function indexes in the first two seconds, such as forced expiratory volume（FEV1） and FEV1/FVC, were compared. Results After treatment, the observation group hs-CRP and Fib, D-D were lower than the control group, the difference was statistically significant（P〈0.05）; after treatment, two groups of forced expiratory volume in 1 second（FEV1）, reduced lung function index FEV1/FVC, but the observation group is still higher than the control group（P〈0.05）. Conclusion Tanreqing injection has high application value in perioperative patients with COPD combined with lung cancer. It can effectively prevent hypercoagulability and reduce lung function damage, which is worthy of promotion.
