摘要
目的研究糖尿病肾病早期微炎症状态与铁蛋白的关系。方法临床纳入77例本院2014年8月~2017年2月期间收治的2型糖尿病患者作为研究对象,另选取同期33例来本院体检中心体检的健康志愿者作为对照组。所有患者根据尿清蛋白排泄率(UAER)分为3组,分别为糖尿病正常清蛋白尿组(NDM组)30例,早期糖尿病肾病组(EDN组)25例,临床糖尿病肾病组(CDN组)22例。检查4组研究对象的血清铁蛋白(SF)和微炎症因子及血糖、血脂等水平,分析糖尿病患者血清铁蛋白与各微炎症因子的关系。结果 4组对象Hb Alc、DBP、HDL-C以及LDL-C差异均无统计学意义;糖尿病患者TC、TG水平比较差异无统计学意义;EDN、CDN组患者FPG、SBP水平明显高于其他两组,差异具有统计学意义(P<0.05)。CDN组SF、hs-CRP、IL-6以及TNF-α水平明显高于EDN组,EDN组高于NDM组,NDM组高于对照组,差异具有统计学意义(P<0.05)。糖尿病患者SF水平与hs-CRP、IL-6以及TNF-α水平呈明显正相关(P<0.05)。结论 2型糖尿病患者SF和微炎症因子水平明显升高,且随着患者UAER的升高,病情加重,SF水平和微炎症因子水平更高,SF和微炎症因子可能与糖尿病肾病的发生发展有密切关系。
Objective To study the correlation of SF(serum ferritin) with the early micro-inflammation state for patients with diabetic nephropathy. Methods 77 patients with type 2 diabetes mellitus treated from August 2014 to February 2017 in our hospital were selected. 33 healthy checkups in our medical examination center were selected as control group. The subjects were divided into NDM(normal diabetic micro-albuminuria)group(30 cases), EDN(early diabetic nephropathy) group(25 cases) and CDN(clinical diabetic nephropathy) group(22 cases) according to the UAER(urinary albumin excretion rate). The index of SF, micro-inflammation factor, blood glucose and blood lipid for four groups were examined;the correlation of SF with the micro-inflammation factors was analyzed. Results The index of Hb Alc, DBP, HDL-C and LDL-C for four groups were not significantly different; the index of TC and TG for diabetes mellitus patients were not significantly different; the index of FPG and SBP for EDN group and CDN group were significantly higher than those of control group and NDM group(P〈0.05); the CDN group had the highest levels of SF, hs-CRP, IL-6 and TNF-α, then followed by EDN group, NDM group and control group(P〈0.05); the index of SF for diabetes mellitus patients had a positive correlation with the level of hs-CRP, IL-6 and TNF-α(P〈0.05). Conclusion For type 2 diabetes mellitus patients, the index of SF and the micro-inflammation factors are greatly increased. As the UAER is increased, the patient's illness condition is worsened with the higher SF level and micro-inflammation factors; it is seen that the SF level and the micro-inflammation factors may have a close correlation with the progression of diabetic nephropathy.
