消痛膏外敷治疗湿热蕴结型痛风性关节炎临床疗效

Clinical Effect of Gouty Arthritis of Damp-Heat Obstruction Treated with External Application of Xiaotong Plaster

目的 ：观察消痛膏外敷对于痛风性关节炎湿热蕴结型患者的治疗效果。方法 ：选择60例痛风性关节炎患者,随机分成治疗组、对照组各30例。治疗组外敷消痛膏治疗,对照组外搽扶他林乳膏。从临床综合疗效、单项临床症状、实验室数据及不良反应等方面进行比较,评价消痛膏的安全性及临床疗效。结果：与治疗前相比,两组临床症状、血尿酸、白细胞计数、超敏C反应蛋白（CRP）值均有明显改善,差异有统计学意义（P〈0.05）;组间比较,降低血尿酸方面疗效相当,差异无统计学意义（P〉0.05）;降低白细胞计数及CRP值方面,治疗组疗效更为明显,差异有统计学意义（P〈0.05）;治疗组和对照组的总有效率分别为87.7%和73.3%,治疗组疗效优于对照组,差异有统计学意义（P〈0.05）;对照组出现两例不良反应,治疗组无。结论 ：外敷消痛膏可以迅速缓解急性期痛风性关节炎患者的临床症状和炎症性指标,降低血尿酸水平,无不良反应,是治疗急性痛风性关节炎的一种安全高效便捷手段。

Objective：To observe the clinical effect of the Xiaotong plaster on treating gouty arthritis with damp-heat obstruction. Methods：Sixty patients with gouty arthritis were randomly divided into the treatment group and control group. In the treatment group patients were treated with external application of Xiaotong plaster,while in the control group patients were treated with the external application of voltaren cream. Two groups were compared on the clinical effect,symptom score and laboratory examination to evaluate the safety and effects of Xiaotong plaster. Results：Compared with before treatment,both groups showed improvement on the clinical symptom scores,serum uric acid,leukocyte count,hypersensitive C-reactive protein（CRP）,with statistical difference（P〈0.05）. Both group could reduce serum uric acid without significant difference（P〉 0.05）.The treatment group showed better effect on reducing leukocyte count and CRP,with statistical difference（P 〈0.05）. The total clinical efficacy effective rate was 87.7% in the treatment group and 73.3% in the control group,with statistically difference（P〈0.05）. There was two cases with adverse reactions in the control group while no case with adverse reactions in the treatment group. Conclusion：External application of Xiaotong plaster could relieve the clinical symptoms and inflammatory indexes rapidly and decrease serum uric acid,without side-effect. It is a safe and efficient method to treat acute gouty arthritis.
