摘要 目的探讨不同剂量维生素D对早产儿钙磷代谢的影响。方法选取2015年2月~2016年2月本院入住的100例早产儿,按照随机数字表法分为高剂量组与低剂量组各50例,在早产儿10日龄时对高剂量组每天给予口服800 U维生素D,对低剂量组每天给予口服400 U维生素D。在20日龄、40日龄时检测早产儿血清中钙、磷和25-羟维生素D的含量。结果 20日龄高剂量组早产儿的钙、磷、25-羟维生素D水平为(2.42±0.12)mmol/L、(1.82±0.53)mmol/L、(45.56±12.45)nmol/L,明显高于低剂量组的(2.03±0.13)mmol/L、(1.71±0.59)mmol/L、(42.51±9.27)nmol/L(P<0.05);40日龄高剂量组早产儿的钙、磷、25-羟维生素D水平为(2.56±0.14)mmol/L、(1.96±0.23)mmol/L、(73.46±18.57)nmol/L,与40日龄低剂量组的(2.12±0.21)mmol/L、(1.81±0.39)mmol/L、(64.48±15.37)nmol/L比较,差异有统计学意义(P<0.05)。结论维生素D能改善早产儿钙磷代谢状态,800 U效果优于400 U。 Objective To investigate the influence of different doses of vitamin D on calcium and phosphorus metabolism in preterm infants. Methods 100 preterm infants admitted in our hospital from February, 2015 to February, 2016 were selected and randomized into high-dose group and low-dose group by using random number table, each had 50 cases. At the age of 10 days, infants in high-dose group were given vitamin D 800U per day by oral administration and infants in low-dose group were given vitamin D 400U per day by oral administration. The levels of serum calcium, phosphorus and 25-hydroxy vitamin D at the age of 20 and 40 days were assessed. Results The levels of alcium, phosphorus and 25-hydroxy vitamin D at the age of 20 days in high-dose group were(2.42±0.12)mmol/L, (1.82±0.53)mmol/L and (45.56±12.45)nmol/L respectively, which were significantly higher than those in low-dose group (2.03±0.13)mmol/L, (1.71±0.59)mmol/L and (42.51±9.27)nmol/L (P〈0.05). The levels of alcium, phosphorus and 25-hydroxy vitamin D at the age of 40 days in high-dose group were(2.56±0.14)mmol/L, (1.96±0.23)mmol/L and(73.46±18.57) mmol/L respectively, which were significantly different from those in low-dose group(2.12±0.21)mmol/L, (1.81±0.39)mmol/L and(64.48± 15.37)nmol/L (P〈0.05). Conclusion Vitamin D could improve the calcium and phosphorus metabolism in preterm infants. The effectiveness of 800 U was better than that of 400 U.
