冠心病合并频发室性早搏患者窦性心率震荡的检测分析

Detection and Analysis of Sinus Rhythm Concussion in Patients with Coronary Heart Disease Complicated with Premature Ventricular

目的分析探讨冠心病合并频发室性早搏(PVC)患者窦性心率震荡(HRT)检测情况,为临床诊治冠心病提供依据。方法随机从我院2016年6月至2017年6月接诊的PVC患者中,选出128例纳入试验中,将合并冠心病的67例患者纳入观察组,将单纯PVC的61例患者纳入对照组,所有患者接受24h动态心电图检测,并对比分析两组患者室早后震荡初始(TO)和震荡斜率(TS)的平均值。结果观察组TO与TS异常率明显高于对照组,观察组TO和TS均异常的患者明显多于对照组(P<0.05);观察组TO明显高于对照组,TS和LVEF明显低于对照组(P<0.05);TO与病变冠脉呈正相关,TS与病变冠脉呈负相关。结论冠心病合并PVC患者的HRT存在明显异常现象,HRT异常在预测心脏性猝死、病变程度中有一定价值。

Objective To analyze the detection of sinus rhythm of heart rate turbulence （HRT） in patients with coronary heart disease （PVC） complicated with frequent premature ventricular contractions （PVC）, and to provide evidence for the clinical diagnosis and treatment of coronary heart disease. Methods Randomized from 128 patients admitted to our hospital from June 2016 to June 2017, 67 patients were included in the observation group and 61 patients with simple PVC Control group, all patients underwent 24h Holter monitoring, and com-parative analysis of the two groups of patients before the shock initial （TO） and the mean value of the oscillation slope （TS ） . Results The abnormal rates of TO and TS in the observation group were significantly higher than those in the control group. The TO and TS abnormalities in the observation group were significantly more than those in the control group（ P 〈 0.05 ） . The TO in the observation group was significantly higher than that in the control group, and TS and LVEF were significantly lower Control group（P 〈 C0.05） . TO was positively corre-lated with coronary artery disease, while TS was negatively correlated with coronary artery disease. Conclusion HRT in patients with coronary heart disease complicated with PVC is obviously abnormal. HRT abnormalities have certain value in predicting sudden cardiac death and degree of disease.
