摘要 目的探讨超声在胎儿心功能检测中的应用价值。方法回顾性分析重庆市大足区人民医院2013年1月至2015年8月产前筛查孕妇160例,经胎儿超声心动图确诊的心脏结构异常者80例作为观察组,同期胎儿心脏结构正常孕妇80例作为对照组,对比分析两组心脏功能指标以及心脏形态指标。结果观察组胎儿心室射血时间为(148.91±6.54)ms,心室心肌做功指数(Tei指数)为0.44±0.05,对照组胎儿心室射血时间为(158.22±7.09)ms,心室Tei指数为0.36±0.06,组间比较差异有统计学意义(P<0.05)。观察组胎儿左心室收缩功能以及舒张功能均较对照组胎儿降低,差异有统计学意义[心脏指数:(389±47)m L·min-1·kg-1比(532±63)m L·min-1·kg-1;二尖瓣口E/A:(0.47±0.07)比(0.57±0.08),t=2.752、2.414,均P<0.05]。观察组胎儿左心室周长和面积以及室间隔厚度与对照组相比,差异有统计学意义(P<0.05)。结论产前超声检查胎儿心脏收缩及舒张功能,可评估胎心发育情况,可作为临床医生诊断的重要指标。 Objective To explore the application value of ultrasound in the detection of fetal heart function. Methods One hundred and sixty cases of pregnant women given prenatal screening from January 2013 to August 2015 in The People’s Hospital of Dazu district were retrospectively analyzed. Eighty cases of abnormal curdiac structure confirmed by fetal echocardiography were selected as observation group and 80 cases of pregnant women with normal fetal heart structure as control group. Cardiac function indexes and cardiac morphological indexes of two groups were compared and analyzed. Results In the observation group,ventricular ejection time was(148. 91 ± 6. 54) ms,the ventricular Tei index was 0. 44 ± 0. 05,while in the control group ventricular ejection time was(158. 22 ±7. 09) ms,the ventricular Tei index was 0. 36 ± 0. 06;there was statistically significant difference between the two groups(P 〈 0. 05).The systolic function and diastolic function of the left ventricle in the observation group were significantly lower than those in the control group;the difference was statistically significant [(CI:(389 ± 47) m L · min-1·kg-1 vs(532 ± 63) m L · min-1·kg-1;E/A:(0. 47 ± 0. 07) vs(0. 57 ± 0. 08),t = 2. 752,2. 414,all P 〈 0. 05]. Compared with the control group,the left ventricular circumference and area and ventricular septal thickness of the fetuses in observation group were significantly different(P 〈 0. 05). Conclusions Prenatal ultrasound examination of fetal cardiac systolic and diastolic function can be used to assess fetal heart development,and it also can be used as an important reference for clinical diagnosis.
