摘要
目的:分析心率变异性与老年高血压并靶器官损害的相关性。方法:选择高血压患者200例作为研究对象,根据年龄分为老年组及非老年组,每组内依据是否合并靶器官损害分亚组,每个亚组50人;选择同期体检健康老人及中年人各50例作为空白组。每亚组患者均进行动态心电图检测,获取心率变异性各参数(SDNN、SDANN、r MSSD、PNN50)的相关值,分析各亚组之间心率变异性各参数的变化,并进行组间比较。结果:SDNN、SDANN、r MSSD、PNN50,老年及非老年组内比较,健康对照组及单纯高血压亚组均低于高血压靶器官损害亚组,健康对照组低于单纯高血压亚组(P<0.05);两组间各对应亚组比较,老年组各亚组均较非老年组对应亚组低(P<0.05)。结论:老年高血压患者的心率变异性较健康老年人下降,老年高血压并靶器官损害患者的心率变异性进一步下降,年龄、高血压及靶器官损害对心脏自主神经功能损害有协同作用。
Objective:To investigate correlation between Heart Rate Variability (HRV)and target organ damage (TOD) in elderly patients with hypertension. Methods: Two hundreds patients with hypertension were observed and divided into elderly group and non-elderly group according to their ages. Both groups were divided into TOD subgroup and simply-hypertension subgroup according to TOD, and fifty healthy elderly persons and fifty healthy non-elderly persons were recruited as healthy group. Holter in all cases was measured, and the correlation parameter values of HRV (SDNN, SDANN, rMSSD, PNN50)were obtained and analyzed, then compared the deference among groups. Results: SDNN, SDANN, rMSSD, PNNS0 of simply-hypertension subgroup and healthy subgroup were lower than those of TOD subgroup in both elderly group and non-elderly group, and the parameter of healthy subgroup were lower than those of simply-hypertension subgroup (P〈0.05). Compared with the corresponding subgroups of the two groups, the subgroups in the elderly group were lower than those in the non elderly group. Conclusion: The HRV of the elderly patients with hypertension is lower than that of the healthy elderly. HRV in elderly hypertensive patients with target organ damage is further decreased. Age, hypertension and target organ damage play a synergistic role in cardiac autonomic dysfunction.
