糖皮质激素辅助治疗肺炎支原体肺炎患儿的免疫、炎症指标的影响研究 被引量:1

摘要 目的:探讨糖皮质激素辅助治疗对肺炎支原体肺炎患儿的免疫、炎症指标的影响。方法:选择2016年1月-2017年6月我院收治的MPP患儿130例,随机均分成治疗组(n=65)和对照组(n=65)。在常规基础治疗的基础上,对照组患儿接受阿奇霉素治疗,治疗组患儿辅助使用糖皮质激素治疗。记录并比较两组患儿的症状、免疫、炎症指标的变化情况。结果:治疗组患儿的咳嗽消失时间、啰音消失时间、X线浸润影消失时间、退热时间、住院时间均显著低于对照组(P<0.05)。治疗组患儿治疗后的IgA、IgE、IgG水平明显优于治疗前(P<0.05);治疗前后IgA、IgE、IgG水平变化明显好于对照组(P<0.05)。治疗组患儿治疗后的ESR、CRP、IL-2、IL-6水平明显优于治疗前,差异有统计学意义(P<0.05);治疗后的两组间比较,差异有统计学意义(P<0.05)。治疗组治疗后的总有效率显著高于对照组(86.96%vs76.92%,P<0.05)。结论:糖皮质激素辅助治疗肺炎支原体肺炎能缩短患儿的治疗时间,减轻炎症反应,提高机体免疫功能。 Objective: To investigate the effect of glucocorticoid adjuvant therapy on immune and inflammatory markers in children with Mycoplasmapneumoniae pneumonia. Methods : 130 cases of children with MPP admitted in our hospital in January 2016 -2017 June were randomly divided into the treatment group (n=65) and the control group (n= 65). On the basis of routine basic treatment, the children in the control group were treated with azithromycin, and the children in the treatment group were treated with glucocorticoid. The changes in the symptoms, immune and inflammato- ry indexes of the two groups were recorded and compared. Results: The cough disappeared time, rales disappear time, X-ray infiltrating shadow disappear time, fever time and hospitalization time in the treatment group were lower than those in the control group (P〈0.05). Levels of IgA, IgE and IgG in the treatment group were significantly better than those before treatment (P〈0.05). Levels of IgA, IgE and IgG before and after treatment were significantly better than those in the control group (P〈0.05). The levels of ESR, CRP, IL-2 and IL-6 of patients in the treatment group after treatment were significantly better than beforetreatment (P〈0.05) , and the difference between the two groups after treatment was statistically significant (P〈0.05). The total effective rate ( 86.96% ) of the treatment group being treated was significantly higher than the control group (76.92%) with a statistically significant difference (P〈0.05). Conclusion: Glucocorticoid assisted treatment of Mycoplasmapneumoniae pneumonia can shorten the treatment time of children, reduce the inflam- matory response, and improve the immune function of the body.
