摘要
目的 :比较用高频振荡通气法与机械通气法治疗新生儿呼吸衰竭的临床效果。方法 :选取资中县中医院在2016年1月至2016年12月期间收治的70例新生儿呼吸衰竭患儿作为本文的研究对象。根据治疗方法的不同将这些患儿分为高频通气组(n=35)和机械通气组(n=35)。采用高频振荡通气法对高频通气组患儿进行治疗,采用机械通气法对机械通气组患儿进行治疗。然后,观察并比较两组患儿在接受治疗前后其PaO_2(动脉血氧分压)、PaCO_2(动脉血二氧化碳分压)和FiO_2(吸入氧浓度)水平的变化情况以及在接受治疗后其发生不良反应的情况。结果 :接受治疗前,两组患儿PaO_2、PaCO_2和FiO_2的水平相比,差异无统计学意义(P>0.05)。接受治疗后5 h、10 h、15 h、20 h,两组患儿PaCO_2和FiO_2的水平与其接受治疗前相比均有明显的下降,其PaO_2的水平与其接受治疗前相比有明显的升高,且高频通气组患儿PaCO_2和FiO_2水平下降的幅度以及PaO_2水平升高的幅度更为明显,差异具有统计学意义(P<0.05)。接受治疗后,高频通气组患儿不良反应的发生率明显低于机械通气组患儿,差异具有统计学意义(P<0.05)。结论 :用高频振荡通气法治疗新生儿呼吸衰竭的效果明显优于用机械通气法治疗新生儿呼吸衰竭的效果。
Objective: To compare efficacy of high frequency oscillatory ventilation and mechanical ventilation in the treatment of neonatal respiratory failure. Methods: Select 70 cases of neonatal respiratory failure treated in Traditional Chinese Medical Hospital of Zizhong County during January to December 2016 for research object. According to difference of therapy,divide the patients to high frequency group( n=35)and mechanical group(n=35).Treat high frequency group with frequency oscillatory ventilation, treat mechanical group with mechanical ventilation. Then compare PaO_2, PaCO_2,,FiO_2 and occurrence of adverse reactions of 2 groups. Result: Before treatment, the difference of PaO_2,PaCO_2 and FiO_2 between 2 groups have no statistical significant(P〉0.05). In 5 h,10 h,15 h,20 h after treatment, PaCO_2 and FiO_2 of 2 groups are decreased, PaO_2 of 2 groups are increased, and the amplitude variation in high frequency group is more significant(P〈 0.05). After treatment, occurrence of adverse reactions of frequency group is lower than that in mechanical group( P〈0.05).Conclusion: The effect of high frequency oscillatory ventilation is better than mechanical ventilation in the treatment of neonatal respiratory failure.
