解毒通脉方联合湿润烧伤膏治疗糖尿病足患者的疗效观察及对血清VEGF、bFGF、SDF-1α的影响

Clinical Observation on Curative Effect of Diabetic Foot Treated with Jiedu Tongmai Fang(解毒通脉方) Combined with Shirun Shaoshang Gao(湿润烧伤膏)and Its Effects on Serum VEGF,bF GF and SDF-1α

目的:观察解毒通脉方联合湿润烧伤膏治疗糖尿病足的临床疗效及对血清血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(b FGF)、基质细胞衍生因子-1α(SDF-1α)表达水平的影响。方法:选取78例糖尿病足患者,随机分为观察组和对照组,每组各39例。对照组给予降糖、改善局部微循环、抗生素治疗,并外用湿润烧伤膏治疗;观察组在对照组治疗的基础上给予解毒通脉方内服治疗,7 d为1个疗程;治疗3疗程后,评定2组患者的临床疗效,比较2组患者治疗前后创面评分以及血清VEGF、b FGF、SDF-1α表达水平的变化情况。结果:观察组临床总有效率为97.4%,高于对照组的87.1%;治疗后,2组患者的创面评分均明显低于治疗前(P<0.05),观察组低于对照组(P<0.05);治疗后2组患者血清VEGF、b FGF、SDF-1α水平较治疗前均升高(均P<0.05),观察组高于对照组(均P<0.05)。结论:解毒通脉方联合湿润烧伤膏治疗糖尿病足患者具有确切的疗效,能够促进糖尿病溃疡创面修复,提高患者血清VEGF、b FGF、SDF-1α水平是其部分作用机制。

Objective： To observe the curative effect of diabetic foot patients treated with Jiedu Tongmai Fang combined with Shirun Shaoshang Gao and its effects on serum l VEGF, b FGF and SDF-1αlevels. Methods： 78 patients with diabetic foot were randomly divided into the control group and the observation group,39 cases in each one according to the random number table. The routine treatments(Hypoglycemic,improvement of local microcirculation,antibiotic treatment） and Shirun Shaoshang Gao were received in the control group,and the observation group were given Jiedu Tongmai Fang to take orally on the basis of the control group,one course was lasted fro 7 days,they were treated for 3 courses. The curative effect was evaluated,changes of the clinical ulcer scores were compared between the two groups and serum VEGF,b FGF,SDF-1αwere detected. Results： The total effective rate was 97. 4 % in the observation group,which was better than that in the control group(87. 1%）(P 〈0. 05）. The ulcer score of the 2 groups after treatment was significantly lower than that before the treatment(P〈 0. 05）,and which in the observation group was lower than that of the control group(P〈 0. 05）. Serum VEGF,b FGF and SDF-1αlevels in the 2 groups after treatment were raised significantly,those were higher than those before the treatment(all P 〈0. 05）,and which in the observation group were higher than those of the control group(P 〈0. 05）. Conclusion： Jiedu Tongmai Fang combined with Shirun Shaoshang Gao can effectively treated diabetic foot,which can significantly promote the healing of ulcer,raising serum VEGF,b FGF and SDF-1α were part of the mechanism.
