顺阿曲库铵对新生豚鼠股神经复合肌肉动作电位的影响研究

摘要 目的研究顺阿曲库铵对新生豚鼠股神经复合动作电位潜伏期及波幅的影响。方法 100例SD豚鼠随机分配为实验组和对照组,每组50例,制作神经肌电监测模型,实验组豚鼠腹腔注射顺式阿曲库铵,对照组注射等体积的生理盐水,注射时(S0)及注射后每5 min直至45 min(S5-45)刺激股神经,并记录股四头肌复合肌肉动作电位(CMAP),分别计算CAMP的潜伏期、波峰峰值,并取其平均值两组进行比较。结果 S0测量潜伏期及波峰平均值两组比较无统计学差异(P>0.05),S5-S45测量实验组潜伏期及波峰平均值低于对照组(P<0.05),实验组CAMP变化明显呈现三个阶段:诱导期(S0-S5)波峰持续下降,潜伏期延长;抑制期(S5-S40)波峰缓慢下降,潜伏期进一步延长;恢复期(S40-S45)波峰逐渐上升,潜伏期缓慢恢复,但仍比给药前低;而对照组用药前后CMAP变化不大(P>0.05)。结论顺阿曲库铵能够对新生豚鼠股神经复合肌肉动作电位产生影响,且具有一定的规律性,相比于传统肌松药,不会完全阻滞肌电的传播,可以作为新型肌松药应用于临床,避免手术对神经的损伤。 Objective To study on the effect of cisatracurium on guinea pig femoral nerve compound muscle action potential. Methods Totally 100 SD guinea pigs were randomly assigned to the experimental group and the control group,50 cases in each group. A neural EMG monitoring model was developed,the guinea pigs in the experimental group were injected with cisatracurium,the control group was injected with physiological saline of equal volume. The femoral nerve was stimulated at the time of injection(S0) and every 5 min until 45 min(S5-45) after injection. And the compound muscle action potential of the four biceps femoris muscle was recorded,the latencyand peak value of CAMP were calculated respectively,and the average values were compared between the two groups. Results S0 measurement latency and wave average,there was no statistical difference between the two groups(P〉0. 05). The mean latency and wave crest of experimental group in S5-S45 measurement were lower than that in control group(P〈0. 05). The change of CAMP in experimental group was obvious in three stages: the period of induction(S0-S5) decreased continuously,prolonged incubation period; the inhibition period(S5-S40) decreased slowly,the incubation period was further extended; The peak of recovery phase(S40-S45) gradually increased and the latent period recovered slowly,but it was still lower than that before administration; The control group before and after medication,CMAP change little(P〉0. 05).Conclusion Cisatracurium can affect the guinea pig femoral nerve compound muscle action potential,and it has a certain regularity. Compared with the traditional muscle relaxant,it can not completely block the spread of EMG. It can be used as a new type of muscle relaxant in the clinic,and avoid the damage of the operation to the nerve.
