腹腔镜与小切口手术切除胆囊治疗胆囊结石的疗效对比分析

Comparative analysis of laparoscopic cholecystectomy and small incision cholecystectomy in treatment of gallstone

目的探究对于胆囊结石行胆囊切除术治疗的患者采用腹腔镜和小切口手术两种术式的临床疗效差异比较分析。方法选择2013年1月至2017年1月时间段内至济源市人民医院就诊的86例胆囊结石患者为研究对象,所有患者均行胆囊切除手术治疗,随机将研究对象分为A组(腹腔镜组)与B组(小切口组),于入院3 d内完善相关检查后进行治疗,比较分析两组研究对象手术操作时间、术中出血量、术后下床活动时间、住院时间、术后并发症及随访的治疗满意度与患者的生活质量评价。结果两组研究对象的手术相关数据比较分析显示A组患者的平均术中出血量显著低于B组,并且术后的平均下床活动时间及平均住院时间显示短于B组,而两组研究对象的平均手术操作时间无统计学差异;术后的相关并发症统计情况显示B组患者出现感染、发热、疼痛的比例显著高于A组;术后半年的随访调查结果显示A组研究对象术后患者的满意度显著高于B组,并且生活质量量化评估结果显著优于B组。结论本研究结果初步证实对于胆结石行胆囊切除术的患者腹腔镜相对于小切口手术的优势更加明显,能够在保证治疗效果的同时明显提高患者的治疗满意度和生活质量。

Objective To investigate the clinical efficacy of laparoscopic cholecystectomy and small incision surgery in the treatment of gallstone patients,and compare the differences of the two surgical methods. Methods Totally 86 cases of gallstones patients from January 2016 to January 2017 period of time to our hospital as the research object,all patients underwent cholecystectomy surgery,were randomly divided into study group A(laparoscopic group） and B group(small incision group） in the 3 days of hospitalization after treatment improve the relevant inspection comparative analysis,operation time,operation of the two groups in operative bleeding and postoperative evaluation of quality of life satisfaction and treatment of patients with complications and follow-up activity time,hospitalization time,postoperative. Results The operation data of the two groups comparison analysis showed that the average blood loss in A group were significantly lower than that in group B,and the mean postoperative ambulation time and hospitalization time showed shorter in the B group,while there was no significant difference in the average operation time of the two groups; postoperative complications related to the statistics according to patients of B infection,fever,pain was significantly higher than that in group A; postoperative follow-up survey showed that half of the patients＇ satisfaction with the A study group after surgery was significantly higher than that of B group,and the quality of life assessment results were significantly better than group B.Conclusion The results of this study demonstrated that for cholelithiasis patients with cholecystectomy laparoscopic with small incision surgery more obvious advantages,can guarantee the effectiveness of treatment and improve treatment satisfaction and quality of life of patients.
