三维子宫输卵管超声造影在输卵管妊娠保守治疗后评价输卵管通畅性中的应用价值 被引量:6

Application value of three-dimensional hysterosalpingocontrast sonography in evaluation of tubal patency in patients with tubal pregnancy after conservative treatment

摘要 目的探讨三维子宫输卵管超声造影在输卵管妊娠保守治疗后评价输卵管通畅性中的应用价值。方法选取2015年7月-2017年1月在广州市番禺区何贤纪念医院接受输卵管妊娠保守治疗的53例患者为研究对象,在治疗3个月后依次进行三维子宫输卵管超声造影检查和宫-腹腔镜下术输卵管通液检查。评估两种不同检查方法结果的一致性,以及三维子宫输卵管超声造影检查的敏感性、特异性、阳性预测值和阴性预测值。结果两种检查结果的一致性较高(K=0.767,95%CI为0.660~0.875,P<0.05)。三维子宫输卵管造影检查诊断输卵管通畅情况的敏感性为81.25%,特异性为98.61%,阳性预测值为96.30%,阴性预测值为92.21%。结论三维子宫输卵管超声造影评估输卵管妊娠患者接受保守治疗后的输卵管通畅性的准确度和敏感度均较高,且其具有无创、安全的特点。 Objective To explore the application value of three-dimensional hysterosalpingocontrast sonography(Hy Co Sy) in evaluation of tubal patency in patients with tubal pregnancy after conservative treatment. Methods From July 2015 to January 2017,53 patients receiving conservative treatment of tubal pregnancy in Hexian Memorial Hospital of Panyu District were selected,three-dimensional Hy Co Sy and hydrotubation under hysteroscope and laparoscope were performed under three months of treatment. The consistency of these two examination methods was evaluated. The sensitivity,specificity,positive predictive value,and negative predictive value of three-dimensional HyCo Sy were calculated. Results The consistency of these two examination methods was high(K = 0. 767,95% CI: 0. 660-0. 875,P 〈0. 05). The sensitivity,specificity,positive predictive value,and negative predictive value of three-dimensional Hy Co Sy in diagnosis of tubal pregnancy were 81. 25%,98. 61%,96. 30%,and 92. 21%,respectively. Conclusion The accuracy and sensitivity of three-dimensional Hy Co Sy in evaluation of tubal patency in patients with tubal pregnancy after conservative treatment are high,and the method has the advantages of non-invasion and safety.
