胎儿纤维连接蛋白检测对妊娠合并细菌性阴道病孕妇早产的预测价值

The Predictive Value of Fetal Fibronectin for the Risk of Preterm Birth in Pregnancy Women with Bacterial Vaginosis

摘要 目的 探讨胎儿纤维连接蛋白在妊娠合并细菌性阴道病孕妇早产预测中的价值.方法 采用酶联免疫吸附法(ELISA)对100例无细菌性阴道病及100例合并有细菌性阴道病的孕妇宫颈阴道分泌物中FFN进行测定,随访其妊娠结局.结果 妊娠合并有细菌性阴道病孕妇中FFN阳性率(73%),显著高于无细菌性阴道病的孕妇(12%)(χ2=78.900,P=0.000).在妊娠合并细菌性阴道病孕妇中,FFN检测阳性孕妇较FFN检测阴性孕妇有更高的风险发生早产(χ2=12.879,P=0.000).FFN检测预测妊娠合并细菌性阴道病孕妇早产发生的敏感度、特异度、阳性预测值和阴性预测值分别是89.6%、42.3%、58.9%、81.5%).结论 宫颈阴道分泌物中FFN与妊娠合并细菌性阴道病有关;FFN检测有助于预测细菌性阴道病相关性早产的发生. Objective To study the predictive value of fetal fibronectin for the risk of preterm birth in pregnancy women with bacterial vaginosis (BV). Methods cervical and vaginal fetal fibronectin in pregnant women without bacterial vaginosis and with bacterial vaginosis were detected by enzyme_linked immunosorbent assay (ELISA), which would be followed for the pregnancy outcomes. Results The positive rate(73%) of FFN in pregnant women with bacterial vaginosis was significantly higher than that (12%) in pregnant women without bacterial vaginosis (χ2 = 78. 900,P = 0.000). Fetal fibronectin-positive women with bacterial vaginosis had higher risk of preterm birth than Fetal fibronectin-negative women with bacterial vaginosis (χ2 = 12. 879, P = 0. 000). The sensitivity, specificity, positive predictive value and negative predictive value of the FFN test predicting preterm birth in pregnant women with bacterial vaginosis were 89.6% ,42.3%, 58.9% and 81.5% respectively. Conclusion Cervical-vaginal fetal fibronectin is strongly related to bacterial vaginosis, it can well predict the occurrence of preterm birth in pregnancy women with bacterial vaginosis and is a powerful predictor for preterm birth in pregnancy women with bacterial vaginosis.
