疏通矫正手法对痉挛型脑性瘫痪患儿GMFM、痉挛肌群表面肌电图、日常生活能力、肌张力评定的影响

Effect of Shutong Jiaozheng Technique Maneuver on GMFM,Surface Electromyogram,Daily Life Ability and Muscle Tension in Children with Spastic Cerebral Palsy

目的:观察疏通矫正手法对痉挛型脑性瘫痪(以下简称痉挛型脑瘫)患儿粗大运动功能评估(gross motor function measure,GMFM)、痉挛肌群表面肌电图、日常生活能力、肌张力的影响。方法:136例痉挛型脑瘫患儿随机平均分为对照组和观察组。两组患者均给予常规基础疗法;同时观察组患者给予疏通矫正手法治疗,对照组患者给予Bobath法及Vojta法治疗。两组均以4周为1个疗程,3个疗程后比较两组患者临床疗效、GMFM、痉挛肌群表面肌电图、日常生活能力、肌张力的变化。结果:治疗前,观察组GMFM总分、日常生活能力的Barthel指数、表面肌电图及肌张力与对照组比较,差异均无统计学意义(P>0.05)。治疗后观察组GMFM总分、日常生活能力的Barthel指数、表面肌电图及肌张力与对照组比较,差异均有统计学意义(P<0.05)。结论:疏通矫正手法治疗儿童痉挛型脑瘫疗效显著。

Objective： To observe the impact of Shutong Jiaozheng Techinque on gross motor function measure(GMFM）,facial electromyography,abilities of daily life and muscle tone in children with Spastic cerebral palsy. Methods： 136 children with cerebral palsy were randomly divided into control group and observation group. Both groups of patients were given conventional basic therapy. Besides,patients in the observation group were given treatment with Shutong Jiaozheng Techinque,and patients in the control group were treated with the Bobath method and Vojta method. Both groups took 4 weeks as a course of treatment. After 3 courses of treatment,the clinical efficacy of GMFM,surface electromyography of diaphragmatic muscle,daily living ability and muscle tension were compared between the two groups. Results： Before treatment,there were no significant differences in the GMFM score,Barthel index,surface electromyography,and muscle tone between the observation group and the control group(P 〉 0. 05）. After treatment,the total score of GMFM,Barthel index,surface electromyography and muscle tension in the observation group were significantly higher than those in the control group(P 〈 0. 05）. Conclusion： The Shutong Jiaozheng Techinque in treatment of spastic cerebral palsy in children is significant.
